Visage is the closest overall match for The Mortuary Assistant’s psychological-scare formula. It combines first-person isolation, repetitive investigation, an ordinary setting made threatening, sanity pressure and defenseless encounters. Choose SIGNALIS instead if you want denser storytelling, cosmic dread, puzzles and scarce resources. SOMA, Silent Hill 2, MADiSON and Layers of Fear are strong alternatives, but each shifts the balance between simulation, exploration, combat, puzzles and jump scares.
This ranking focuses on the things that make The Mortuary Assistant unusually effective: mundane tasks that leave your attention exposed, supernatural investigation, unreliable reality, isolation, vulnerability and scares that arrive after tension has already accumulated.

Why The Mortuary Assistant feels different from ordinary horror games
The Mortuary Assistant begins with a practical premise. You have completed a mortuary-science degree and start an apprenticeship at River Fields Mortuary. Your work involves embalming and corpse-care procedures, but the mortuary is also a site of supernatural investigation: you must identify demonic activity and perform a banishment.
That combination creates a distinctive rhythm. You are not simply walking through a haunted house waiting for the next scripted shock. You are completing familiar, procedural work while watching for signs that reality has changed. The tasks occupy enough attention to make a sudden event intrusive, yet they are repetitive enough to leave room for dread. As Brian Clarke, the game’s creator, put it: “It’s critical it’s not too engaging.”
#1 Best Overall

- Bloody Arsenal of Weapons: From chainsaws to cleavers, pick up a variety of weapons to turn your enemies into bloody pulp. Relive the golden age of gaming with a glorious chiptune soundtrack that perfectly complements the retro aesthetic.
- Multiple Game Modes: With 6 different game modes, whether you're looking for a quick beat 'em up session or an extended challenge, there's a mode that fits your style.
- Languages: English, French, German, Italian, Portuguese (Brazil), Spanish (LATAM), and Spanish (Spain) in game text.
Steam’s listing tags the game as Horror, Supernatural, Demons, Mystery, Atmospheric, Singleplayer, Surreal, Simulation, First-Person, Exploration, Thriller, Hidden Object and Medical Sim. It released on 2 August 2022 from developer DarkStone Digital and publisher DreadXP. A Steam snapshot accessed on 18 October 2026 showed 92% positive English reviews from 4,841 reviews; that is a time-specific audience signal, not a permanent quality score.
The best alternatives therefore need more than ghosts and jump scares. They should reproduce at least one of the following: a vulnerable first-person viewpoint, a slow-burn atmosphere, a task loop that keeps you exposed, psychological destabilization, environmental clues, or an uncertain boundary between the supernatural and the protagonist’s mind.
Quick comparison: which game should you pick?
| Game | Best for | Primary scare mechanism | Core loop | Vulnerability and pacing | How closely it matches |
|---|---|---|---|---|---|
| Visage | Closest overall mood | Sanity erosion, grief, family trauma and reality distortion | Explore a changing house, inspect clues and use items | Defenseless, slow-burn tension; light helps preserve sanity | Closest first-person psychological equivalent, though it replaces mortuary work with house exploration |
| SIGNALIS | Cosmic and identity horror | Memory, identity, uncertainty and cosmic dread | Puzzles, exploration and scarce-resource management | Vulnerable survival-horror pacing with deliberate resource pressure | Less similar in setting, stronger for narrative density and interpretation |
| SOMA | Philosophical dread | Consciousness, identity and existential fear | Explore and uncover the story | Atmospheric tension rather than simulation-focused tasks | Best when the psychological questions matter more than the work routine |
| Silent Hill 2 | Guilt, grief and symbolism | Emotional trauma and unreliable reality | Explore, solve puzzles and survive combat encounters | Traditional survival-horror structure | Excellent thematic match, but less like a first-person workplace simulator |
| MADiSON | Paranormal investigation and sharper shocks | Possession, occult imagery and unstable reality | Camera-led investigation and puzzle progression | More aggressive scare cadence than Visage | Strong for supernatural investigation, weaker for quiet procedural tension |
| Layers of Fear | Guided haunted-house storytelling | Mental breakdown, distorted spaces and unreliable narration | Linear environmental exploration and story reveals | Authored, cinematic pacing | Best for art-directed atmosphere rather than open-ended task pressure |
Review figures in this table should be read as snapshots. Steam showed 85% positive English reviews from 4,072 reviews for Visage and 96% from 21,711 reviews for SIGNALIS on 18 October 2026. Percentages and review totals change over time and do not measure scare intensity directly.
1. Visage — the closest first-person psychological match
What makes it similar
Visage is explicitly described as a first-person psychological horror game set in a mysterious, ever-changing house. Its pace is deliberately slow and atmospheric. You investigate rooms, follow environmental clues and interpret surreal images while dark entities stalk the building. You are defenseless, and maintaining sanity is central to survival; staying in light helps prevent the character from going insane.
That design reproduces the emotional geometry of The Mortuary Assistant: an isolated interior, ordinary objects made suspicious, a routine of checking and rechecking spaces, and the knowledge that you cannot simply overpower what is hunting you. Both games make the player study the environment for small inconsistencies before a supernatural event becomes undeniable.
How the loop differs
Visage does not simulate embalming, corpse care or demon identification. Its loop is house exploration, item interaction, clue interpretation and sanity management. The changing architecture can make progress feel uncertain, whereas The Mortuary Assistant gives you a recognizable work process that is repeatedly disrupted.
Who should choose it
- Choose Visage if you want first-person isolation and a defenseless protagonist.
- Choose it if slow-burn tension is more important than frequent jump scares.
- Choose it if you enjoy reading psychological trauma through spaces and objects.
- Skip it if you specifically want realistic workplace procedures or a clearly structured shift-based loop.
2. SIGNALIS — the best choice for cosmic, identity and memory horror
Why it works for Mortuary Assistant fans
SIGNALIS is described as classic psychological survival horror in a dystopian future. Its retrotech world follows a technician searching for a lost partner and lost dreams while uncovering terrifying secrets. The game emphasizes challenging puzzles, scarce resources, cosmic dread, identity, memory and the terror of the unknown.

- Each chapter features engaging new toy characters that can be either friend or foe
- Survive and progress by solving challenging puzzles
- Each game reveals to you a newly discovered area of the massive toy factory
- The Poppy Playtime Triple Pack includes a reversible coversheet and a Poppy Playtime sticker sheet
Its strongest connection to The Mortuary Assistant is not its setting but its uncertainty. In both games, you must decide what details matter, whether a disturbing event has a rational explanation, and how much confidence to place in your own interpretation. The pressure comes from incomplete information as much as from direct attacks.
What changes compared with The Mortuary Assistant
SIGNALIS is more overtly narrative and systemically survival-horror focused. Resource scarcity affects your decisions, and puzzles are a central activity rather than an occasional interruption to a work routine. The science-fiction setting and identity themes make it more abstract, while The Mortuary Assistant’s fear grows from a recognizable mortuary becoming contaminated by the supernatural.
Who should choose it
- Pick SIGNALIS for dense lore, existential questions and symbolism.
- Pick it if inventory pressure and puzzle solving are part of your ideal horror loop.
- Expect less realistic simulation and more deliberate interpretation of memory and identity.
3. SOMA — philosophical dread instead of procedural scares
SOMA is repeatedly grouped with The Mortuary Assistant in psychological-horror recommendations because it sustains fear through ideas as much as through creatures. Its strongest appeal is existential: questions about consciousness, identity and what makes a person remain psychologically threatening long after an individual encounter ends.
The game is a good fit when you want atmosphere and narrative discovery rather than a detailed task simulation. Exploration reveals the premise gradually, allowing uncertainty to accumulate. The tension is usually philosophical and environmental instead of being built around embalming-like procedures or a constant sequence of jump scares.

4. Silent Hill 2 — guilt, grief and symbolic psychological horror
Silent Hill 2 is the strongest alternative for readers who want psychological horror rooted in guilt, grief, symbolism and an unreliable reality. Its emotional focus makes the setting feel like an external expression of the protagonist’s inner life, a quality shared with the way The Mortuary Assistant turns a workplace into a mirror for fear and trauma.
The structure is more traditional survival horror. You explore, solve puzzles and face survival-combat situations rather than performing a repeated professional procedure. That makes the pacing more varied and the player less consistently defenseless. The payoff is a more character-driven story with extensive symbolic interpretation.

- Grace Ashcroft, an FBI intelligence analyst who is introverted and easily scared, representing a new type of character for the Resident Evil series. Grace will experience horror from the same perspective as the player as she learns to overcome her fears throughout the course of the story.
- For the first time ever in Resident Evil history, players will be able to freely switch between both first- and third-person perspectives throughout the game.
- An Anniversary to Remember: With March 2026 marking the Resident Evil series’ 30-year anniversary, Resident Evil Requiem marks its own milestone in the franchise.
- Pre-orders for Resident Evil Requiem are now open, with the bonus of Grace’s costume: Apocalypse. Please note that the picture weapon does not come with the costume, but it can be obtained in-game.
If you want the same first-person, isolated feeling, Visage is closer. If you want a psychologically layered story in which guilt and grief drive the horror, Silent Hill 2 is the better direction.
Because editions and platforms can change, verify the current remake or edition and its regional availability on an official store page before purchase.
5. MADiSON — paranormal investigation with a more aggressive scare rhythm
MADiSON suits players who want occult investigation, possession imagery and a reality that becomes increasingly unstable. Its camera-led investigation and puzzle progression give the player a concrete tool for examining supernatural evidence, which makes it a natural recommendation for anyone who enjoyed searching the mortuary for signs that something is wrong.
The major difference is cadence. MADiSON generally aims for more direct paranormal pressure and more frequent shocks, while Visage relies more heavily on prolonged quiet and uncertainty. It is therefore a better choice if you want the investigation to produce immediate danger rather than letting tension sit in the background.
Do not assume that its current platforms, editions or price match older recommendation lists; check the current official listing for your region.
6. Layers of Fear — guided, art-focused haunted-house horror
Layers of Fear is an adjacent choice for players who prefer authored environmental reveals, distorted architecture and a guided cinematic route through a haunted space. Its horror is built around mental breakdown, unreliable narration and rooms that change as the story advances.
Compared with The Mortuary Assistant, it offers less simulation and less open-ended investigation. You are following a designed sequence of revelations rather than choosing how to balance professional tasks with supernatural diagnosis. That makes it accessible for players who want atmosphere and visual storytelling without managing a large set of systems.

For repetitive tasks interrupted by supernatural events
Start with The Mortuary Assistant itself, then try Visage. Visage’s item checks and environmental investigation do not imitate embalming, but they preserve the feeling of concentrating on a small action while something impossible develops nearby. MADiSON is the next choice if you want that investigative idea with more abrupt shocks.

Visage is the clearest match. Its entities stalk the house while your main tools are observation, movement, light and interpretation. SIGNALIS also creates vulnerability through scarce resources, although its survival-horror systems make the pressure more item-management oriented.
For cosmic uncertainty and identity questions
Choose SIGNALIS first and SOMA second. SIGNALIS combines puzzles, resource pressure and a surreal science-fiction setting; SOMA puts more weight on philosophical dread and the implications of consciousness.
For guilt, grief and symbolic storytelling
Choose Silent Hill 2. Its horror is more character-centered and symbolic than The Mortuary Assistant’s procedural setup, but it addresses the same interest in psychological damage becoming part of the environment.
For frequent paranormal shocks
Choose MADiSON. It is the most direct option in this list for occult imagery and an aggressive scare cadence. If you prefer tension that accumulates gradually, Visage is a better fit.

Best Value
- A dark, enthralling 4-5 hour narrative to keep you hooked and guessing until the end. Gorgeous visuals as you traverse numerous and diverse environments, presented with a cinematic, film-like aesthetic. Ranged and melee combat with upgradeable weapons to help you survive your nightmarish journey.
- A stellar voice cast, starring Valerie Rose Lohman (What Remains of Edith Finch, Wolfenstein: Youngblood), Jonah Scott (Dying Light 2: Stay Human, Date Everything), and Aleks Le (Persona 3: Reload, Marvel Rivals, Ghost of Yotei).
- A beautifully dynamic and haunting soundtrack composed and performed by Jacob Noska, and Jonah Henthorne: members of the band Auric Echoes.
- The puzzles you encounter are more than your standard key hunt. Project Songbird rewards those with an eye for detail, making each puzzle a challenge of creativity and wits.
- The combat of Project Songbird offers a variety of approaches inspired by the classics of the survival horror genre. Melee and gunplay is slow and weighty, and ammo is incredibly scarce. However, combat isn't your only option, as you have a multitude of tools at your disposal to aid in sneaking past any creatures that wish to do you harm.
How the six games compare on the important design axes
Scare psychology
The Mortuary Assistant uses anticipation, uncertainty and the collision between professional routine and demonic intrusion. Visage shifts that toward sanity erosion, grief and family trauma. SIGNALIS uses identity, memory and cosmic uncertainty. SOMA makes existential questions the main source of dread. Silent Hill 2 uses guilt and grief, MADiSON uses possession and occult instability, and Layers of Fear uses mental breakdown and unreliable narration.
Task loop
The reference game is unusual because its task loop is recognizable work: corpse care, embalming-related actions and checking evidence. Visage substitutes exploration and item interaction. SIGNALIS substitutes puzzles and resource management. SOMA focuses on exploration and narrative discovery. Silent Hill 2 combines exploration, puzzles and combat. MADiSON uses camera-led investigation and puzzles. Layers of Fear follows linear environmental exploration.
Player vulnerability
Visage offers the closest defenseless experience. SIGNALIS makes you vulnerable through limited resources and survival-horror consequences. Silent Hill 2 is less helpless because combat is part of its structure. MADiSON and Layers of Fear emphasize escape, investigation or guided progression rather than a sustained professional-task simulation.

Pacing
Visage is the strongest slow-burn choice. The Mortuary Assistant alternates routine with sudden supernatural disruption. SIGNALIS and Silent Hill 2 provide more conventional survival-horror progression, while MADiSON generally delivers shocks more often. Layers of Fear controls the pace most tightly because its environmental changes are authored along a guided route.
Narrative density
For environmental implication, Visage is closest. For explicit lore, puzzles and thematic interpretation, SIGNALIS is the strongest. SOMA and Silent Hill 2 are especially suitable when the protagonist’s identity and emotional history are central to the horror. MADiSON and Layers of Fear put more emphasis on paranormal or artistic presentation.
A practical decision guide
- Ask whether the setting matters more than the psychology. If you need a first-person domestic or workplace-like space, choose Visage. If you are comfortable with a major setting change, SIGNALIS or SOMA can deliver deeper existential horror.
- Decide how much control you want. For defenseless tension, choose Visage. For resource decisions, choose SIGNALIS. For combat-inclusive survival horror, choose Silent Hill 2.
- Choose your scare cadence. Select Visage for quiet dread, MADiSON for faster paranormal shocks, or Layers of Fear for a guided cinematic rhythm.
- Choose your narrative emphasis. Select SIGNALIS for identity and memory, SOMA for consciousness, Silent Hill 2 for guilt and grief, and Visage for environmental implication.
- Verify the current edition before purchase. Platform, edition and pricing details for SOMA, Silent Hill 2, MADiSON and Layers of Fear are not fixed by this guide. Use the current official store page for your region.
Bottom line: the best order to try them
Start with Visage if what you loved was The Mortuary Assistant’s isolated first-person perspective, defenselessness, ordinary spaces turning hostile and slow psychological escalation. Try SIGNALIS next if you want more puzzles, resource pressure and cosmic identity horror. Move to SOMA for philosophical dread, Silent Hill 2 for guilt and grief, MADiSON for aggressive paranormal investigation, and Layers of Fear for guided art-focused haunted-house storytelling.
