The best starting point for tailbone (coccyx) pain is usually a pressure-relief cushion with a rear U-shaped cutout or a posterior wedge, sized so it fits your chair without upsetting your posture. NHS guidance recommends using a specially designed coccyx cushion while sitting and supporting the lower back. Mayo Clinic notes that some people find a wedge-shaped cushion most helpful. Shape, height, firmness and stability all affect whether a cushion actually feels better, so treat the choice as a fit problem rather than a universal product ranking.
What kind of cushion is best for tailbone pain?
Choose a cushion designed to unload the back of the pelvis, not a flat seat pad. A rear opening or tapered section is intended to reduce direct contact beneath the coccyx while the surrounding foam supports the buttocks and thighs. The most useful shape depends on where your pain is, the chair you use and how the cushion changes your sitting position.
- Start with a U-shaped coccyx cushion if you want the most direct relief behind the tailbone. The notch removes material at the rear of the seat.
- Consider a wedge-shaped cushion if changing the angle of your pelvis feels better or if the rear cutout of a U-shape does not match your painful area. Mayo Clinic specifically says some people find a wedge most helpful.
- Use a doughnut cautiously. Its central opening can reduce pressure in one spot, but the raised ring may concentrate load around the painful area or feel unstable. It is not automatically equivalent to a coccyx cushion.
No cushion can be called best for every person. A model that relieves pressure in an office chair may be too tall for a car seat, and a firm cushion that supports one person may feel unforgiving to another. The buying decision should therefore begin with your main seat, your pain location and the amount of height you can add without losing a neutral posture.

When you sit, pressure is shared among the sitting bones, thighs and the soft tissues around the pelvis. A coccyx cushion changes that contact pattern. A U-shaped rear notch or posterior wedge leaves less material directly under the coccyx, while the remaining foam carries more of the load through the buttocks and thighs. The goal is pressure reduction, not correction of the underlying cause of pain.

The effect is highly individual. The coccyx may hurt at its tip, along one side, or after a particular injury or prolonged sitting posture. A cutout that is too short, too narrow or positioned too far forward can miss the painful spot. Likewise, a cushion can feel comfortable at first but create new pressure if it tilts you backward or raises your knees above the normal desk or pedal position.
NHS advice is to use a specially designed coccyx cushion when sitting and to maintain good posture with lower-back support. A cushion works best as part of that setup: sit back against the chair, keep the lower back supported, and avoid slumping onto the rear edge of the seat.
U-shaped, wedge or doughnut: comparing the main designs
| Design | Where pressure is removed | Useful starting point | Potential fit issue |
|---|---|---|---|
| Rear U-notch | A U-shaped opening at the back leaves the coccyx area less supported directly beneath it. | People who want a clear rear relief zone while keeping a mostly level seat. | The notch may not line up with your painful spot, and the cushion can feel too narrow if your hips extend beyond its side bolsters. |
| Wedge or posterior wedge | A tapered profile changes the sitting angle and reduces contact at the rear of the pelvis. | People who feel better with a slight forward or altered pelvic angle; Mayo Clinic says some people find this shape most helpful. | Too much taper can alter desk height, steering-wheel reach or pedal control. |
| Doughnut | A central hole removes contact from the middle of the seat. | Only when the opening and ring match your pressure pattern and the cushion remains stable. | The raised ring can transfer pressure to its edge, feel unstable, or fail to unload the coccyx at the rear. |
Look at the opening from above and compare it with the area that actually hurts. A rear U-notch should sit behind the coccyx rather than under the middle of your thighs. On a wedge, check which end is higher and whether that orientation is intended for the seat you are using. A doughnut should not force you to balance on the rim.

Get the dimensions and height right
Dimensions are not cosmetic. They determine whether the cushion supports your weight over enough surface area and whether it fits inside the chair without sliding or hanging over an edge. Measure the usable width and depth of the seat, including the space between armrests or side bolsters. For a vehicle, measure the seat pan rather than the overall upholstery, and check that the cushion does not interfere with the seat belt buckle.

Width and depth
A cushion should leave your thighs supported without pushing your hips against the chair arms. If it is too short front-to-back, the rear opening can sit in front of the coccyx; if it is too deep, the front edge may press behind your knees. The sides should remain on the seat when you shift your weight, rather than bridging a gap or curling upward.
Height and taper
Every added inch changes your hip, knee and elbow angles. At a desk, a tall cushion can make the keyboard feel low and encourage shoulder elevation unless the chair or desk height can be adjusted. In a car, it can bring your head closer to the roof, change your view over the dashboard or alter pedal reach. In a fixed-height chair, a lower profile may be safer even if a taller cushion feels softer.
As a concrete reference, OPTP lists its Coccyx Pillow at 17.5 by 14.25 inches, with height tapering from 3 inches to 1.25 inches. Those dimensions describe that product; they are not a universal size standard. Compare them with your seat before buying and confirm which end is meant to face the rear.

Check the complete sitting system
- With the cushion in place, both feet should rest securely on the floor or vehicle footwell.
- Your knees should not be forced markedly higher than your hips unless that position is deliberately comfortable and does not affect control.
- Your lower back should still contact the chair’s support instead of leaving a gap that makes you slump.
- At a desk, your forearms should reach the keyboard without shrugging; in a vehicle, steering and pedal reach must remain natural.
How firm should a tailbone cushion be?
Look for support that resists bottoming out without feeling like a hard board. The cushion must keep its relief channel open under your weight; if the foam collapses until you contact the chair, the cutout no longer does its job. Conversely, excessive firmness can create pressure at the edges of the notch or under the thighs.

A practical firmness check
- Place the cushion on your normal chair and sit with your back supported.
- After several minutes, notice whether you are resting on the foam or on the chair beneath it. A distinct hard spot suggests bottoming out.
- Check the rear opening and its borders. Sharp pressure at the notch edge means the shape or firmness may be wrong even if the center feels soft.
- Stand and sit again. The foam should recover its shape rather than remaining visibly compressed.
Foam type, density and layering vary by model, and labels such as “soft” or “orthopedic” do not provide a consistent measurement across sellers. Prioritize a clearly described support core, a shape that stays open under load and a return policy that lets you exchange an unsuitable firmness.
Materials, cover and stability features
Foam construction
Premium or molded foam can hold a defined notch or taper more consistently than a loosely filled pad, but marketing language alone does not establish durability. Look for the manufacturer’s description of the foam and its intended support. Avoid a cushion that feels permanently flattened after a short trial or that shifts its relief opening as you move.
Washable cover
A removable, washable cover is valuable because the cushion sits against clothing every day. Check whether the cover is machine washable, which cycle and temperature the maker specifies, and whether it must be air-dried. OPTP documents a machine-washable mock-mesh cover on its Coccyx Pillow. Other products may use non-removable covers or different cleaning instructions, so do not assume the same care method.
Non-slip behavior
A stable base keeps the opening aligned with your coccyx. Test the cushion on the actual upholstery: leather, vinyl and smooth fabric can behave differently. A non-slip underside, grippy fabric or sufficient friction can help, but a separate mat may change the cushion’s height and should not create a tripping edge. If the cushion slides backward when you sit, repositioning it repeatedly is a sign that the setup is not working.

A buying process that avoids the common mistakes
- Identify your primary seat. Choose the office chair, vehicle seat or other surface where you spend the most time. A cushion that fits one seat may not fit another.
- Mark the painful area. While seated briefly, note whether discomfort is directly at the rear center, off to one side or spread across the sitting bones. Use that observation to judge the location and width of the cutout.
- Measure the seat. Record usable width, depth and the height you can add while keeping your feet, desk or pedals in a safe position.
- Select geometry. Start with a rear U-notch for direct coccyx unloading or a wedge if a changed pelvic angle feels better. Treat a doughnut as a specific fit experiment, not a default.
- Check support details. Confirm the foam construction, cover removal and washing instructions, and whether the base is stable on your upholstery.
- Read the return terms before opening the package. Pressure distribution is personal, and marketplace policies change. Verify the time window, condition requirements and whether return shipping is charged.
- Trial it in short sessions. Begin with the seat and posture you normally use. Increase sitting time only if symptoms are not worsening and the cushion remains aligned.
Set up the cushion correctly
Office or home chair
- Place the cushion fully on the seat, with its rear opening toward the chair back unless the manufacturer’s instructions specify another orientation.
- Sit back so your pelvis is supported rather than perched on the front edge.
- Adjust chair height so your feet are supported and your elbows can reach the work surface without lifting your shoulders.
- Use the chair’s lower-back support. If the cushion pushes you too far forward to contact it, the cushion may be too deep or too tall for that chair.
- Change position regularly. A cushion reduces pressure while sitting; it does not make uninterrupted sitting harmless.
Vehicle seat
Before driving, check that the cushion cannot slide, does not cover the seat-belt buckle and leaves full control of the pedals and steering wheel. Confirm that your view and head position are unchanged enough for safe driving. If the added height or tilt affects control, do not use that cushion in the vehicle; choose a lower profile or another seating solution.
Symptoms that indicate a poor match
- Pain at the front edge: the cushion may be too short, too deep or tilted so that it presses behind your knees.
- Pressure beside the notch: the opening may be too narrow, or the foam may be too firm at its borders.
- No relief at the tailbone: the cutout may be in the wrong position, the cushion may be facing the wrong way, or you may be bottoming out.
- New low-back or hip discomfort: the height or angle may be changing your posture beyond what your chair can accommodate.
- Sliding: the base or cover may not grip your upholstery, making alignment inconsistent.
Stop using a model that increases pain, numbness or instability. Recheck orientation and chair height once; if the problem remains, use the return or exchange process rather than trying to adapt your body to a poor fit.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Returns and trial periods matter
Two cushions with identical dimensions can feel different because their foam, cover friction and cutout edges differ. You cannot reliably judge those variables from a product photograph. Read the return policy before purchase, especially when buying through a marketplace whose terms can change. Confirm whether the item must be unused, whether the cover can be removed for inspection, the deadline for requesting a return and who pays shipping.

When a cushion is not enough
A cushion addresses sitting pressure; it does not diagnose why the coccyx hurts. Pain can follow an injury, prolonged pressure or another condition that needs assessment. NHS guidance says to seek medical help if the pain has not improved after a few weeks or affects daily activities. Mayo Clinic also advises evaluation when coccyx pain persists.
Seek prompt medical advice for severe or rapidly worsening pain, pain after a significant fall, fever, new weakness or numbness, or changes in bladder or bowel control. Those symptoms require medical assessment rather than experimentation with different cushions. Even without warning signs, arrange an evaluation if you cannot sit, sleep or work normally despite reducing pressure.
A documented example of the specifications to compare
OPTP’s Coccyx Pillow illustrates the details worth checking on any listing: a stated footprint of 17.5 by 14.25 inches, a taper from 3 inches to 1.25 inches, premium foam and a machine-washable mock-mesh cover. These are the documented specifications for that example, not evidence that it will fit every chair or suit every body. Use the same checklist—dimensions, height profile, foam support, cover care and stability—when comparing other cushions.
Quick decision checklist
- Does the shape remove pressure where your coccyx actually hurts?
- Will the width and depth stay entirely on your seat?
- Can you add the cushion’s full height without compromising feet, desk ergonomics or vehicle control?
- Does the foam support you without bottoming out or creating hard edges?
- Is the cover removable and washable according to clear instructions?
- Will the base stay aligned on your upholstery?
- Can you return or exchange it if the pressure pattern is wrong?
- Have you planned medical follow-up if pain persists or limits daily activities?
Bottom line
For most shoppers, a properly sized rear U-shaped coccyx cushion is the sensible first trial, with a wedge-shaped design as the main alternative when a changed sitting angle feels better. Choose by pressure location, dimensions, height, firmness, stability and return terms—not by shape alone. Use it with lower-back support and good posture, and seek medical advice when pain persists or interferes with normal activities.
