For a mild reaction limited to the sting area, move away from more stings, remove any visible stinger promptly, wash the skin, apply a wrapped cold compress, and raise the affected arm or leg. Hydrocortisone cream, calamine, or an oral antihistamine may ease local itching; follow the product label and local professional advice. Call your local emergency number immediately for trouble breathing or swallowing, swelling of the face or throat, faintness, or widespread symptoms. If the person has a prescribed epinephrine autoinjector, use it as directed and seek emergency care.
What swelling and itching can mean
Swelling, redness, pain, and itching around the sting can be a local reaction. It may look worse before it gets better, so the size of the swelling alone does not tell you whether a reaction is dangerous. A large local reaction can extend well beyond the sting site and still be local; however, symptoms elsewhere in the body need more urgent attention.
Mayo Clinic describes mild reactions as typically involving immediate sharp pain, a welt, and swelling that resolves within hours. A moderate reaction can include itching or flushing, with swelling that worsens over the next day or two and may last up to seven days. The American College of Allergy, Asthma & Immunology (ACAAI) says large local swelling often peaks two to three days after a sting and may last a week or longer. These are different descriptions of reaction patterns, not a single guaranteed timeline. A previous sting reaction also does not reliably predict the severity of the next one. Mayo Clinic; ACAAI

- Get to a safe place. Move away from the area to reduce the chance of more stings.
- Remove a visible stinger promptly. Scrape or brush it sideways with a fingernail or the edge of a card. NHS guidance advises against using tweezers because squeezing the stinger may release more venom. NHS
- Wash the site. Clean the skin with soap and water.
- Remove nearby rings or tight jewelry. Do this before swelling makes them difficult to remove.
- Cool the area. Apply a cold compress wrapped in cloth; do not put ice directly on the skin. Mayo Clinic suggests 10–20 minutes, while NHS advises at least 20 minutes for swelling. Mayo Clinic; NHS
- Raise an affected limb if practical, to help with swelling.
Options for easing itch or pain
Hydrocortisone cream or calamine may help with local itching and swelling. An oral antihistamine may help itching, but some products cause drowsiness. A nonprescription pain reliever may help pain if it is appropriate for the person. Follow the package directions and check with a pharmacist or clinician about age restrictions, other medicines, or health conditions that could make a product unsuitable. These options are for local symptoms; they do not treat anaphylaxis. Mayo Clinic; NHS

When a sting is an emergency
Call your local emergency number immediately for possible anaphylaxis or another severe reaction. Warning signs include trouble breathing or swallowing; swelling of the lips, tongue, face, or throat; fainting; or widespread hives or other symptoms affecting more than the sting site. Dizziness alongside other symptoms is also concerning. Do not wait to see whether these symptoms pass, and do not rely on an antihistamine pill to treat anaphylaxis.
If the person has an epinephrine autoinjector prescribed for them, use it as directed and get emergency care even if symptoms improve. Emergency numbers and care pathways vary by country. Mayo Clinic; ACAAI; NHS

When to get prompt or non-emergency medical advice
Not every concerning sting needs an ambulance, but some situations warrant prompt advice. Follow the care pathway where you live; the NHS examples below are UK-specific, while Mayo Clinic guidance is US-based.
Do these 3 things before closing this tab:
1Scan for outdated or missing drivers - takes under a minute2Clear out junk files and repair common Windows errors3Fix the driver behind crashes, sound loss and screen glitchesSeek prompt advice for higher-risk situations
- A sting in the mouth or throat, or near an eye.
- Multiple stings. Mayo Clinic specifically advises immediate care for multiple stings in children, older adults, and people with heart or breathing problems.
- Worsening or non-improving symptoms, or symptoms such as vomiting with abdominal pain, dizziness, fever with swollen glands, or a previous serious allergic reaction. NHS advises urgent GP or NHS 111 help for these situations.
Ask about possible infection or symptoms that persist
Contact a clinician if the area seems infected, symptoms persist, or the person feels unwell. NHS says a pharmacist can advise about an area that is hot, red, painful, swollen, or producing pus or fluid. Mayo Clinic advises arranging a healthcare visit if symptoms have not gone away within three days or if other allergic symptoms occurred. These are source-specific care recommendations, not a universal deadline. NHS; Mayo Clinic

Follow-up after a severe reaction
After emergency treatment for a severe sting reaction, arrange medical follow-up if it has not already been planned. Mayo Clinic says referral to an allergy specialist may follow. An allergist can assess whether further evaluation or prevention is appropriate; venom immunotherapy is a specialist treatment for selected people with insect-sting allergy, not a self-directed treatment.
