No. Newer tests have changed how laboratories identify bacteria, but they have not made culture obsolete. Culture grows organisms from a specimen and can yield a viable isolate for susceptibility testing and further study. MALDI-TOF can rapidly identify many cultured organisms, while culture-independent tests can detect pathogen-associated targets without first growing the organism. The right method depends on what information is needed, not simply which test is fastest.
How do labs identify bacteria?
Laboratories use different methods to answer different questions: whether a pathogen or its genetic or antigen target is present, which organism is present, whether it is susceptible to particular antimicrobials, and how it relates to other cases. A workflow may combine methods rather than rely on a single test.
In a conventional culture workflow, a laboratory processes a specimen to grow organisms. It can then identify an organism from the resulting growth and use the isolate for additional testing. Because the organism is viable material, culture can support antimicrobial susceptibility testing as well as antigenic, genetic, and other characterization. The value of that isolate is especially relevant when clinicians or public-health teams need more than an initial detection.
What is the difference between culture, MALDI-TOF and molecular tests?
| Method | What it detects or produces | What it is useful for | Important limitation |
|---|---|---|---|
| Culture | Growth of organisms from a specimen; may produce a viable isolate. | Identification of the recovered organism and downstream susceptibility testing or characterization. | It depends on recovering growth, and does not by itself answer every identification or resistance question. |
| MALDI-TOF mass spectrometry | A mass-spectrometry profile used to identify a microorganism grown in culture. | Identification of cultured microorganisms within the capabilities of the system and laboratory workflow. | Results depend on the organism, the system’s reference database, and validated procedures; it does not replace the culture step in the FDA-described use. |
| Culture-independent diagnostic test (CIDT) | A pathogen-associated genetic or antigen target in a specimen. | Rapid detection that may be easier to perform than traditional culture. | A positive result generally does not provide a viable isolate or all information used for patient care and public-health practice. |
What MALDI-TOF adds after culture
MALDI-TOF MS is a laboratory identification method, not a home diagnostic device. The FDA describes the regulated device category as systems for identifying microorganisms cultured from human specimens. In practical terms, culture supplies the organism, and MALDI-TOF analyzes its profile to help identify it. The method therefore accelerates or changes the identification stage of a culture workflow; it is not equivalent to directly detecting a pathogen target in an uncultured specimen.

Identification is not automatic or independent of laboratory quality. The result depends on whether the organism can be recognized by the system’s database and on the laboratory’s validated workflow. The Clinical and Laboratory Standards Institute’s M58 standard addresses performance, reporting, quality assurance, user verification, and integration of MALDI-TOF identification into laboratory workflows. A database update or improved workflow can affect results, so an identification rate from one laboratory and period should not be treated as a universal accuracy measure.
What culture-independent tests do—and what they leave out
CDC describes CIDTs as tests that detect a gene or antigen associated with a pathogen. They can be faster, easier, and cheaper to use than traditional culture tests, making them useful when a timely detection result matters. But detecting a target is different from recovering a living organism.

CDC notes that CIDTs generally do not provide some information used in patient care and public-health practice. A result may not show antimicrobial response, and without an isolate it may not provide the same information needed to determine whether a pathogen from one patient is related to pathogens in other cases, as in outbreak surveillance. A positive target result can therefore be important without supplying everything a culture and follow-up testing can offer.

There is no universally best method based on speed alone. The decision depends on the specimen and the clinical or public-health question. If the immediate need is to detect a particular pathogen-associated target, a CIDT may provide a useful rapid answer. If an isolate is needed for susceptibility testing, strain characterization, or further investigation, culture retains a distinct role. MALDI-TOF can help identify organisms recovered by culture, but the laboratory still has to consider database coverage and validated procedures.

- Detection: Is the question whether a specific target is present?
- Identification: Is the goal to name an organism recovered from the specimen?
- Susceptibility: Is information about how the organism responds to antimicrobials needed?
- Characterization and surveillance: Is a viable isolate needed for comparison with other cases or additional testing?
These needs can lead a laboratory to use more than one method. A rapid molecular or antigen result may help answer an initial question, while culture may be pursued to obtain an isolate for further work. The exact approach varies with the specimen, pathogen, and laboratory’s validated testing options; the sources cited here do not establish one diagnostic algorithm for every infection.
A dated example: MALDI-TOF results changed as one lab’s database developed
A 2018 report in CDC’s journal Emerging Infectious Diseases described one clinical microbiology laboratory in Marseille, France. From January 2012 through March 2018, it reported 351,937 nondereplicated bacterial identifications by MALDI-TOF; 28,391, or 8.1%, were unidentified or misidentified overall. The laboratory’s yearly unidentified rate fell from 17.7% in 2012 to 3.6% in 2018, and the article reported correct identification of 744 unique bacterial species.

- Please Note: To reach 10Gbps speed, keep the cable under 3.3 ft. For USB A Male to USB C adapters, try flipping the USB C connector. USB C Male to USB A adapters support bidirectional 10Gbps transfer within 3.3 ft
Those figures document the experience of that laboratory over that period, not a general sensitivity, accuracy rate, or current benchmark for all MALDI-TOF systems. They illustrate why database coverage and laboratory experience matter, but they cannot predict performance for a different organism, system, or workflow.
Culture remains a tool, not a verdict on newer tests
In a 2002 CDC-hosted perspective on emerging bacterial diseases, Pierre Houpikian and Didier Raoult described culture as having “irreplaceable advantages” for antigenic studies, antibiotic susceptibility testing, experimental models, and genetic studies. That statement reflects a historical perspective, not a new guideline, but the underlying distinction remains useful: tests that detect targets and tests that recover organisms provide different kinds of evidence.

Newer methods have expanded the options for bacterial identification and can shorten time to some results. Culture still matters when a viable isolate or downstream information is needed. A laboratory’s choice is therefore best understood as matching the method to the question, rather than choosing between an old method and a replacement.
