Autoimmune encephalitis (AE) requires urgent specialist care. Once infection and major mimics have been reasonably excluded, treatment usually begins with high-dose corticosteroids, often combined with intravenous immunoglobulin (IVIG) or plasma exchange in severe illness. If the response is inadequate, neurologists may escalate to rituximab or cyclophosphamide and, in selected refractory cases, tocilizumab or bortezomib.
Recovery is usually measured in months rather than days. Consciousness, agitation and seizures may stabilize first; memory, speech, executive function, mood, fatigue and independence can improve later, sometimes for years. Some people retain permanent acquired-brain-injury deficits and need prolonged rehabilitation.
Why autoimmune encephalitis treatment is time-critical
Autoimmune encephalitis occurs when the immune system attacks brain tissue or related receptors. Presentations can include rapidly changing behavior or psychiatric symptoms, memory loss, seizures, abnormal movements, reduced consciousness, sleep disruption and autonomic instability. Because infectious encephalitis can look similar and immunosuppression can worsen an untreated infection, clinicians first perform an urgent evaluation rather than assuming every case is autoimmune.

Serum and cerebrospinal-fluid (CSF) antibody tests should be collected when feasible, but waiting for antibody results can cause harmful delays. The 2024 Brazilian consensus recommends beginning treatment within four weeks of symptom onset when clinical suspicion remains high. Basic CSF findings, cultures, viral testing, brain imaging, electroencephalography and other tests are used to reasonably exclude infection, cancer and other major mimics. Suspected infectious encephalitis requires appropriate antimicrobial evaluation and treatment.

The practical principle in the best-practice recommendations is: “Once infection is ruled out based on basic CSF results … start acute immunotherapy with high dose corticosteroids (or IVIG or PLEX if steroids are not preferred or contraindicated).” The treating neurologist or neuroimmunologist must balance urgency against bleeding, clotting, infection and other individual risks.
How specialists select an immunotherapy plan
There is no single regimen that fits every antibody or syndrome. The treatment discussion generally weighs:
- Severity, intensive-care needs, respiratory failure, dysautonomia and status epilepticus.
- Whether the suspected mechanism is antibody-mediated, cell-mediated or paraneoplastic.
- Bleeding risk, low platelet count, kidney or heart disease, hyponatremia, thromboembolism risk and previous reactions.
- Active or possible infection, malignancy risk and vaccination status before deeper immunosuppression.
- Whether an underlying tumor is present and how quickly it can be treated.
- Monitoring capacity, venous access, local expertise, cost and insurance or hospital availability.
- Evidence quality, anticipated relapse risk and the need for maintenance therapy.
A 2021 best-practice survey illustrates the broad clinical consensus rather than a guaranteed protocol: 84% of respondents selected corticosteroids alone or combined with IVIG or plasma exchange as first-line treatment for a general AE presentation, and 80% selected rituximab as the preferred second-line agent when the antibody was unknown.

High-dose intravenous corticosteroids
Intravenous methylprednisolone is the most common initial immunotherapy. It suppresses inflammation quickly and can be started while antibody testing continues. The exact dose, number of treatment days and subsequent oral-prednisone taper depend on the syndrome, response and medical history; the cited guidance does not mandate one universal schedule.
Clinicians monitor glucose, blood pressure, mood or psychosis, gastrointestinal complications, secondary infections and other steroid effects. A person with uncontrolled infection, severe steroid toxicity or a contraindication may receive IVIG or plasma exchange instead of, or before, steroids.

Intravenous immunoglobulin
IVIG supplies pooled antibodies that modulate abnormal immune activity. It is commonly added to steroids in severe disease or used when steroids are unsuitable. Important considerations include kidney function, fluid status, thromboembolic risk, blood viscosity and prior infusion reactions. Infusion monitoring and adequate venous access are required.
Plasma exchange
Plasma exchange (PLEX) removes circulating antibodies and other immune mediators through a series of blood exchanges. It may be chosen for severe or rapidly progressive disease, for a contraindication to IVIG, or when a rapid antibody-removal strategy is desired. Central-line complications, bleeding, low blood pressure, electrolyte changes and infection risk must be assessed.

Combining or sequencing therapies
Canadian guidance recommends high-dose corticosteroids plus IVIG or plasma exchange for severe AE. A critically ill patient may receive steroids and one of the antibody-removal treatments promptly, while a less severe patient may start with steroids alone and be reassessed. The choice between IVIG and PLEX is individualized; neither is automatically superior for every antibody, comorbidity or hospital setting.
Searching for and treating an underlying tumor
In paraneoplastic AE, the immune response is triggered by a cancer elsewhere in the body. Doctors therefore investigate for an underlying neoplasm while immunotherapy proceeds, using examinations and age- and risk-appropriate imaging or other tests. Treating the tumor can be central to stopping the immune trigger, improving neurologic control and reducing future activity. A negative first scan does not necessarily end surveillance when the syndrome and antibody pattern remain strongly paraneoplastic; the specialist sets follow-up according to the suspected cancer.
When first-line therapy does not work
Deciding that escalation is needed
Failure is not defined by one bad day. Teams reassess consciousness, seizures, movement, behavior, autonomic function, CSF and electroencephalography, while checking for infection, medication effects, metabolic problems, missed seizures and cancer. In severe disease, worsening or lack of meaningful improvement after initial therapy commonly prompts second-line treatment. In mild or moderate disease, specialists may allow a longer observation interval before escalating, provided the patient is stable.

Rituximab
Rituximab depletes CD20-positive B cells and is particularly considered in antibody-mediated syndromes such as N-methyl-D-aspartate receptor (NMDAR) encephalitis. It is the preferred second-line option in the 80% survey result described above when the antibody is unknown. Screening for hepatitis and other infections, reviewing vaccines, and monitoring blood counts and immunoglobulin levels are important because infection risk can persist after treatment.
Cyclophosphamide
Cyclophosphamide is a potent immunosuppressant that may be favored when disease appears primarily cell-mediated or paraneoplastic, or when rituximab is unsuitable. Toxicities include low blood counts, serious infection, bladder injury, infertility risk and a small long-term malignancy risk. Hydration, blood-count monitoring and specialist dosing are required.
Rituximab and cyclophosphamide are not interchangeable self-care options. The decision depends on antibody biology, cancer association, pregnancy and fertility considerations, organ function, previous immunotherapy and the local neuroimmunology team’s experience.
Options for refractory autoimmune encephalitis
If substantial disease persists after first- and second-line treatment, selected centers may consider tocilizumab or bortezomib. Tocilizumab blocks interleukin-6 signaling; bortezomib targets antibody-producing plasma cells. Best-practice guidance describes the supporting evidence for these later-line treatments as minimal, largely based on limited series and specialist experience rather than large randomized trials.
Before using either drug, clinicians revisit the diagnosis and look for occult infection, ongoing cancer, seizures, medication toxicity and structural complications. These agents can produce serious infections, blood-count abnormalities, liver or metabolic problems and other toxicities, so treatment generally requires autoimmune-neurology expertise and close laboratory surveillance.

Bridging and maintenance treatment
After acute therapy, some patients receive a prednisone taper, repeated IVIG or methylprednisolone, or a steroid-sparing immunosuppressant. The aim may be to prevent relapse while the immune system settles, to cover a delayed response to rituximab, or to manage a syndrome known to relapse. There is no universal duration: the plan reflects the antibody, relapse history, cancer status, residual inflammation, side effects and comorbidities.
| Therapy stage | Typical role | Main decision points |
|---|---|---|
| Initial | IV methylprednisolone; add IVIG or PLEX for severe disease | Urgency, infection exclusion, ICU status, contraindications and access |
| Second-line | Rituximab or cyclophosphamide | Worsening or inadequate response; antibody mechanism, cancer association and toxicity |
| Refractory | Tocilizumab or bortezomib in selected cases | Minimal evidence, specialist availability and intensive infection/toxicity assessment |
| Maintenance | Prednisone taper, repeat IVIG or methylprednisolone, or steroid-sparing drug | Relapse risk, response, adverse effects and long-term immunosuppression burden |
What recovery usually looks like
Recovery is a sequence rather than a single discharge date. A common pattern is:
- Stabilization of consciousness and agitation: sleep-wake cycles, delirium and severe behavioral disturbance may settle first.
- Control of seizures and movement symptoms: seizures, dyskinesias, rigidity or other abnormal movements can improve as inflammation falls, although intensive-care complications may slow progress.
- Cognitive and communication gains: attention, memory, speech, planning and processing speed often recover more gradually.
- Psychiatric, fatigue and autonomic recovery: anxiety, depression, irritability, sleep problems, exhaustion and blood-pressure or heart-rate instability may continue after other signs improve.
- Return of function: work, school, driving, self-care and social independence require assessment rather than assuming that a normal conversation means full recovery.
Early neurologic stabilization does not equal complete recovery. Plateaus can be followed by later gains. NMDAR encephalitis, in particular, can show progressive cognitive improvement for years after treatment has ended. Conversely, some survivors have permanent memory, executive, movement, psychiatric or physical deficits consistent with acquired brain injury.
Recovery speed, residual disability and relapse frequency vary substantially by AE syndrome and antibody target. Treatment delay, ICU complications, prolonged seizures or status epilepticus, dysautonomia, cancer association, age and the degree of impairment at onset all influence the outlook. A page from the International Autoimmune Encephalitis Society reports 7% mortality and emphasizes that prompt treatment and escalation are associated with better long-term recovery; the figure should not be treated as an individual prediction because risk differs by syndrome and illness severity.
Rehabilitation after the acute illness
Rehabilitation should begin as soon as the person is medically stable and continue across transitions from intensive care to hospital, inpatient rehabilitation, outpatient care and home. The program may include:

Families can help by keeping a symptom and medication timeline, bringing prior records to appointments, using written routines and allowing extra processing time. Driving, returning to hazardous work and reducing supervision should be decided with the treating team after cognition, seizures, movement and judgment have been assessed.
Follow-up, relapse and seizure medicines
Follow-up should evaluate cognition, behavior, seizures, movement, sleep, mood, autonomic symptoms, medication effects and day-to-day function. The Brazilian consensus recommends acute and postacute assessment with functional and cognitive scales. Antibody levels and a single MRI can provide context, but neither should replace a clinical examination and functional assessment.
Relapse risk is syndrome-dependent. New seizures, renewed psychiatric or behavioral change, worsening memory, abnormal movements, unexplained sleep disruption or autonomic symptoms warrant prompt medical contact, especially when immunotherapy is being tapered. Antiseizure medicines are sometimes reduced after the acute stage when seizures were clearly symptomatic and the neurologist judges recurrence risk to be low; stopping them independently is unsafe.

Can autoimmune encephalitis be cured?
Some people achieve near-complete recovery and remain relapse-free, while others have ongoing disability or recurrent disease. “Cure” therefore depends on the syndrome, antibody, cancer status and follow-up period. The realistic goal is rapid control of brain inflammation, treatment of any immune trigger, prevention of relapse and restoration of the highest possible level of cognitive, physical and social function.
