Controller (or maintenance) treatment is taken on a regular schedule to keep asthma controlled and reduce future risk. A reliever is taken when symptoms occur for rapid relief. They may be two separate inhalers, or one prescribed ICS-formoterol inhaler may serve both jobs under an AIR or MART plan. The medicine name, prescribed schedule and your written asthma action plan—not the inhaler’s color—determine which role applies to you.

Controller vs. reliever: the essential difference

Asthma inhalers are assigned roles by what they are intended to do and when the prescription tells you to use them. A controller is ongoing treatment: it is usually taken every day or at another regular interval, including when you feel well. Its purpose is to improve control, prevent symptoms and lower the chance of future flare-ups. A reliever is used when symptoms such as wheeze, chest tightness, coughing or breathlessness occur, with the aim of opening the airways quickly.

The roles do not always match the number of devices. Some people have a daily preventer plus a separate symptom inhaler. Others have one inhaler containing inhaled corticosteroid (ICS) and formoterol that is used both at scheduled maintenance times and as needed. Your prescription and written action plan establish the correct arrangement.

Question Controller or maintenance Reliever
When is it used? Every day or on another scheduled frequency, whether or not symptoms are present As needed when symptoms occur, according to the action plan
Main purpose Improve day-to-day control and reduce future risk Provide quick relief during symptoms
Typical medicine examples Often an ICS-containing treatment; the exact product and dose depend on the prescription A rapid-acting bronchodilator such as a SABA, or an anti-inflammatory reliever containing ICS-formoterol in an appropriate plan
Number of devices May be a separate inhaler or the maintenance part of a combination plan May be a separate inhaler or the same inhaler used for maintenance under MART
How to identify it Read the active ingredients and scheduled instructions Read the active ingredients and the as-needed instructions in the action plan

This is a role-based explanation, not a way to identify an unfamiliar inhaler. If the label, pharmacy instructions and action plan do not agree, ask a pharmacist or prescriber before using it.

What does “controller” or “maintenance” mean?

“Controller” is common patient language for treatment aimed at both current symptoms and future risk, especially regular ICS-containing treatment. The Global Initiative for Asthma (GINA) uses “maintenance” more precisely. Its 2026 strategy report defines maintenance treatment as asthma treatment prescribed for use every day or on a regularly scheduled basis, and explains that the word describes frequency rather than a particular medicine class.

That distinction prevents two common errors. First, an ICS-containing medicine is not automatically a daily controller in every prescription: some ICS-containing relievers are used only when needed. Second, a medicine can have a maintenance role in one plan and a reliever role in another, depending on the prescribed schedule and combination.

The US National Heart, Lung, and Blood Institute (NHLBI) describes long-term control medicines as treatments taken regularly to prevent attacks and control symptoms. Inhaled corticosteroids are among the long-term control medicines it lists. The exact inhaler, dose, age authorization and monitoring requirements vary, so a broad class name cannot replace your prescription label.

Why regular treatment matters even when you feel well

Asthma inflammation and airway sensitivity can persist between noticeable attacks. A scheduled controller is intended to address that ongoing tendency rather than wait for symptoms to appear. Missing doses may leave the underlying condition less controlled even if you have no immediate discomfort. Do not stop or reduce a controller because you feel better unless the clinician who manages your asthma gives you a new plan.

What is a reliever or rescue inhaler?

GINA defines a reliever as an asthma inhaler taken as needed for quick relief of asthma symptoms. Traditional relievers are short-acting beta2-agonists (SABAs), which relax airway muscle quickly. They address the immediate narrowing but do not, by themselves, provide the anti-inflammatory treatment that some people need to reduce future risk.

Some modern reliever plans pair an ICS with formoterol, a bronchodilator that starts working quickly. The ICS supplies anti-inflammatory treatment while formoterol provides rapid bronchodilation. This is not interchangeable with every combination inhaler: the exact product, strength, device and authorization matter.

Frequent reliever use is a signal, not a target

A reliever is useful for symptoms, but needing it more often than your plan allows, waking at night, limiting activities or getting progressively less benefit can indicate inadequate control or an exacerbation. Those changes warrant the action-plan step for review or urgent care; they are not a reason to keep escalating doses on your own.

What’s the difference between a controller and a rescue inhaler?

“Rescue inhaler” is an everyday term usually referring to a reliever. The practical difference is the treatment schedule and objective:

  • Controller: scheduled use to maintain control and reduce future risk.
  • Rescue/reliever: as-needed use for rapid symptom relief.
  • Combination plans: one ICS-formoterol inhaler can perform both roles when the prescription specifically uses an anti-inflammatory reliever (AIR) or maintenance-and-reliever therapy (MART).

Color is not a reliable identifier. Inhaler colors differ between countries, manufacturers and devices, and a familiar color does not prove an inhaler is suitable for your plan. Use the named active ingredients, the pharmacy label and your written instructions.

Can one inhaler be both a preventer and a reliever?

Yes, but only under a regimen designed for that purpose. In MART, the same ICS-formoterol inhaler is taken at scheduled maintenance times and additional doses are taken as needed for symptoms. In an AIR approach, an ICS-formoterol inhaler is used when symptoms occur; the exact schedule and step depend on the guideline pathway and prescription.

GINA’s adult and adolescent pathways illustrate the overlap. Its preferred Track 1 uses as-needed low-dose ICS-formoterol at lower treatment steps and uses the same class for scheduled maintenance plus as-needed doses at higher steps. Track 2 uses a SABA or an ICS-SABA reliever instead. These are guideline pathways, not a universal instruction to change medicines, and local product licensing can differ.

When one inhaler must not be used as the reliever

GINA cautions that ICS-formoterol should not be used as the reliever when the scheduled maintenance combination contains a different long-acting beta2-agonist (LABA). The NHS also notes that not every inhaler containing formoterol is prescribed as MART. Formoterol in the ingredient list is therefore not enough to establish that an inhaler is an AIR or MART device.

Never infer a one-inhaler plan from a similar-looking product, a previous prescription or another person’s treatment. Confirm the exact name and instructions with your prescriber or pharmacist.

Do I need both a controller and a reliever inhaler?

Some people do; some do not. The answer depends on the plan selected for your asthma, the severity and pattern of symptoms, age, local approvals and how well control is maintained.

Plan pattern What the person may have How roles are assigned
Separate-device plan A scheduled preventer plus a separate symptom reliever Use the preventer on schedule and the reliever only as the action plan directs
AIR plan An ICS-formoterol inhaler used when symptoms occur The inhaler is an anti-inflammatory reliever; there may or may not be scheduled doses at the prescribed step
MART plan One ICS-formoterol inhaler The same device is used for scheduled maintenance and additional symptom relief
Other combination plan A maintenance inhaler containing a non-formoterol LABA and a separate reliever Use the separately prescribed reliever; do not substitute ICS-formoterol unless the clinician has prescribed it that way

Having two inhalers does not mean one is optional, and having one inhaler does not mean every dose is interchangeable. Keep the written plan with the medicines and ask for clarification after any brand, device or dose change.

How to tell which role your inhaler has

  1. Read the dispensing label. Look for the active ingredients, strength, number of puffs and whether the instruction says scheduled, as needed or both.
  2. Check the written action plan. It should name the inhaler to use for routine treatment, symptoms, worsening asthma and emergency escalation.
  3. Match the device to the prescription. Metered-dose inhalers, dry-powder inhalers and soft-mist devices have different loading and breathing steps. A device that looks similar may operate differently.
  4. Ask before substituting. A pharmacy substitution, a new strength or a different device can change technique and may change whether the product is authorized for AIR or MART.
  5. Record questions rather than guessing. Bring every inhaler, spacer and written instruction to an asthma review so the clinician can reconcile them.

Your written asthma action plan is the decision guide

NHLBI describes an asthma action plan as a written document covering triggers, signs of an attack, which medicines to take and when, when to call a healthcare professional or go to emergency care, and whom to contact. It converts general inhaler roles into instructions for your medicines.

What a useful plan should specify

  • The exact name and appearance or device description of each prescribed inhaler.
  • Routine maintenance doses and timing.
  • Which inhaler to use for symptoms and the maximum use allowed in the plan.
  • Signs that asthma is worsening, such as increasing symptoms, reduced activity or poor response to the reliever.
  • When to contact the prescriber, an urgent service or emergency services.
  • Relevant triggers, allergies, peak-flow instructions if prescribed, and emergency contact details.

Keep the plan current after a medication change, pregnancy, a hospital visit, a significant flare-up or a change in device. Make sure family members, school or workplace contacts and caregivers know where it is when appropriate.

What to do when symptoms worsen

Use the reliever specified by your own plan, not a generic internet dose. NHS patient guidance gives different response instructions for a traditional blue reliever and for AIR or MART inhalers, so those directions should not be merged. Follow the plan’s stated maximum dose and escalation step.

Technique, spacers and device differences

Correct medicine in the wrong technique may deliver too little drug. Ask a clinician or pharmacist to watch you use the device and to recheck technique after a switch. Dry-powder inhalers generally require a forceful, deep inhalation; press-and-breathe metered-dose inhalers require coordination, and some are used with a spacer. Follow the instructions supplied for your specific device.

A spacer chamber is an accessory compatible with some inhalers. NHS guidance mentions using a spacer with a blue reliever during an attack if one is available. Compatibility and cleaning instructions vary. A spacer is not a medicine, does not replace the prescribed inhaler and should not be assumed to fit every device.

Common mistakes and how to correct them

Using the reliever as the only asthma treatment

Short-term symptom relief can mask a need for anti-inflammatory treatment. The NHS advises against relying on a blue reliever on its own. Arrange a review if you have no controller plan, are using the reliever frequently or are unsure what your preventer is.

Taking a controller only when symptoms appear

That changes a scheduled treatment into an unplanned reliever strategy. Check the label and action plan; do not alter the frequency without professional advice.

Assuming every ICS-formoterol inhaler is interchangeable

Products differ in dose, device, age approval and whether they are prescribed for AIR or MART. A formoterol-containing maintenance combination with a different LABA arrangement may require a separate reliever.

Using color as the prescription

Colors are not standardized internationally. Identify the medicine by active ingredients and written instructions.

Sharing or borrowing inhalers

Inhalers are prescribed for an individual diagnosis, dose and device. Do not share them or use someone else’s rescue inhaler as a substitute for medical assessment.

When to request an asthma review

Contact the clinician managing your asthma when reliever use is increasing, symptoms interfere with sleep or normal activity, inhaler technique is uncertain, side effects appear, or the device is difficult to operate. A review can check diagnosis, triggers, adherence, technique, the action plan and whether the current step remains appropriate.

Bring all inhalers, spacers, medication lists and peak-flow records if you use them. Ask the clinician to write down whether the plan is separate-device, AIR or MART, which inhaler is used for each situation, and the emergency threshold.

Age, country and product approvals matter

GINA recommendations, NHS pathways and NHLBI patient guidance describe established approaches but do not authorize every product in every country. Indications can differ by age, inhaler strength, formulation and regulator. The fact that a guideline discusses ICS-formoterol does not establish that every ICS-formoterol product is approved for your age group or for reliever use where you live.

Use the instructions supplied by your local prescriber and regulator. If you move countries, change insurance or receive a substitute device, have the regimen checked rather than assuming the previous role carries over.

Questions to ask your clinician or pharmacist

  • Which inhaler is my scheduled controller or maintenance medicine?
  • Which inhaler should I use when symptoms start?
  • Is my plan AIR, MART or a separate preventer-and-reliever plan?
  • What is the maximum reliever use in one day before I seek help?
  • Which symptoms mean I should call urgently or use emergency services?
  • Is this exact device approved for my age and prescribed role?
  • Should I use a spacer, and is it compatible with this inhaler?
  • Can you watch my technique and write the steps in my action plan?

If you cannot answer those questions from your label and action plan, pause before changing how you use the inhaler and obtain professional clarification.
