Sex differences in the nervous system do not come from a single source. Sex-chromosome genes can act directly in neural cells; genes also guide gonadal development, which changes steroid-hormone exposure. Hormones organize circuits during sensitive developmental windows and modulate them later, while sensory, social and behavioral experience continually remodels those circuits. The balance differs by species, trait, age and context, and human male–female distributions usually overlap substantially rather than forming two discrete brain types.
The short answer: three interacting causes
It is misleading to ask whether nervous-system sex differences are caused by DNA, hormones or nurture as if only one mechanism can be responsible. A more accurate model is a chain of interacting influences:
- Chromosomal genes provide molecular instructions in every cell, including neurons and glia. X- and Y-linked genes can differ in dosage or expression, and some X-linked genes escape the usual X-inactivation process.
- Gonadal hormones alter development after genes influence gonadal formation. Steroid hormones can organize neural circuits during critical periods and activate or modulate them later in life.
- Experience—including sensory input, learning, stress, social relationships and behavior—changes synapses and circuit activity through plasticity.
These mechanisms are coupled rather than competing explanations. A gene can make a receptor, hormone-producing enzyme or plasticity-related protein; a hormone can change which genes are transcribed; and experience can stabilize or remodel the circuit that resulted. Therefore, the same hormone exposure can produce different outcomes in animals with different genotypes, at different ages or in different environments.

Direct action in neural cells
X and Y chromosomes are not merely an on–off switch for testes or ovaries. Their genes can be expressed directly in neural cells and affect cell survival, migration, synapse formation, neurotransmitter systems and other processes. Arthur P. Arnold’s unified organizational theory review notes that some sex differences are not explained by gonadal hormones but instead reflect the primary action of genes encoded on the sex chromosomes.

Dosage is one route. Most cells with two X chromosomes silence one X chromosome in each cell, but a subset of X-linked genes escapes complete inactivation. Those genes can therefore be expressed at different levels in XX and XY cells. Y-linked genes have no equivalent copy on the X chromosome, so their products can also create cellular differences before, during or independently of hormone exposure.
Indirect effects through gonadal development
Many genes influence the formation and function of the gonads. The resulting differences in gonadal hormones then affect the nervous system. In this pathway, the initial cause is genetic, but the neural mediator is endocrine. Separating those steps is essential: an experiment that changes chromosome complement may alter both direct neural gene activity and hormone production unless the two are controlled independently.
Genes also set the sensitivity of a circuit to hormones. Receptor abundance, local enzymes and signaling proteins determine whether a given steroid has a large, small or even opposite effect in a particular region. This is why “genetic” and “hormonal” explanations often describe different links in the same biological pathway.

Organizational effects during development
The classic organizational–activational framework distinguishes two broad kinds of steroid action. During sensitive developmental windows, hormone exposure can produce relatively lasting changes in circuit structure and function. Steroids regulate transcription and can alter synapse formation, receptor expression, neurotransmitter systems and intrinsic excitability. The timing of exposure matters because a circuit may be receptive only while particular cells, receptors or downstream enzymes are present.
“Lasting” does not mean irreversible. It means that development has been shifted in a way that can persist after hormone levels fall. Later experience, injury, disease or additional hormones may still modify the resulting circuit.

Activational effects in adulthood
In mature nervous systems, changing hormone levels can produce more reversible or state-dependent effects. Receptors are found not only in cell nuclei but also at membranes, synapses, mitochondria and other cellular sites. Hormone fluctuations can therefore alter neuronal excitability, synaptic transmission, motivation and stress responses on timescales ranging from minutes to days, while also influencing longer-term gene transcription.
An adult effect may depend on an earlier organizational change. A circuit organized to be hormone-sensitive during development can respond strongly to a later rise or fall in steroid levels, whereas another circuit may respond weakly because it lacks the relevant receptor or metabolic machinery.

Testosterone is not always the final mediator
It is also too simple to equate “male-typical” development with direct testosterone action. In some developing neural tissues, testosterone is converted by the enzyme aromatase into estradiol. Estradiol then produces organizational effects through estrogen receptors. Local metabolism, receptor distribution, dose and timing determine which steroid actually acts on a given cell.
This conversion illustrates why hormone experiments require careful design. Removing a gonad, adding testosterone or blocking an estrogen receptor can affect several linked pathways at once. Results cannot automatically be generalized to every brain region, developmental stage or species.
How experience reshapes hormone-sensitive circuits
Plasticity during critical periods
Experience is biologically active, not a layer added after development is complete. Sensory systems are tuned by the patterns of input they receive, and cognitive and emotional circuits are modified by learning, stress and social interaction. During critical or sensitive periods, synapses may be especially easy to create, remove or strengthen. Hormones can change the timing or threshold of these periods, while experience determines which available connections are retained.

Social and behavioral feedback
Experience can also alter the hormonal environment itself. Activity, nutrition, stress and social conditions influence endocrine signals, which then feed back onto neural circuits. A behavior that initially differs between groups may generate different practice histories; those histories can amplify, reduce or reverse a circuit difference without changing the underlying chromosomes.
Perineuronal nets and stabilization
Recent work points to extracellular structures called perineuronal nets as possible brakes on plasticity. As these structures develop, they can stabilize established connections and make large developmental changes less likely. If hormones or experience alter when such constraints appear, they may help explain why the same learning opportunity has different effects at different ages.
Genes, hormones and experience compared
| Influence | Typical timing | Biological level | Reversibility | What changes its effect | Human causal evidence |
|---|---|---|---|---|---|
| Sex-chromosome genes | Any stage, including before gonadal hormone production | Chromosome dosage, gene expression and cellular development | Usually persistent at the cell-lineage level, though downstream circuits remain plastic | Which genes escape X-inactivation, cell type, receptor and enzyme expression | Hard to isolate from hormone and social effects |
| Gonadal steroids | Strong organizational effects in developmental windows; activational effects throughout life | Transcription, membranes, synapses, mitochondria and circuit excitability | Developmental effects can persist; adult effects are often state-dependent | Timing, dose, receptor distribution and local metabolism such as aromatization | Clinical and natural experiments are informative but rarely separate every pathway |
| Experience | Especially powerful during sensitive periods, but continues across adulthood | Synapses, network activity, learning and extracellular stabilization | Often modifiable, although stabilized circuits can resist change | Prior learning, stress, social setting, age and hormone state | Correlations are common; controlled causal separation is limited |
The table describes tendencies, not three independent boxes. A chromosome can alter hormone sensitivity; a hormone can gate learning; and learning can change later endocrine and neural responses.
What human studies can—and cannot—establish
Average differences are probabilistic
Human imaging and clinical studies find average sex-associated differences in some brain measures and in the risk of some disorders. These findings generally involve overlapping distributions: many individuals in each group fall within the other group’s range. A group average therefore cannot identify an individual’s chromosome complement, hormone history, abilities or likely behavior.
Structural measurements are particularly sensitive to sample size, age, endocrine state, medication, scanner and analytic method. A clinical review emphasizes that inconsistencies in human structural assessments reflect this methodological complexity and the need for findings that generalize across populations. A result observed in adolescents, for example, may not describe adults with different hormone levels or developmental histories.

Correlation rarely identifies the mechanism
If a scan finds a group difference, the observation alone cannot determine whether it arose from sex-chromosome genes, prenatal or pubertal hormones, accumulated experience, or interactions among them. Social treatment and opportunities can differ between groups, and those differences can affect the same neural systems being measured. Human studies can test associations, follow developmental trajectories and examine unusual endocrine conditions, but they rarely manipulate chromosomes, gonads, hormones and experience independently.
The field has often omitted sex as a variable
Interpretation is also limited by study design. A 2011 review reported that more than 50% of neuroscience studies published in 2009 used only male subjects or did not specify sex. A 2024 Nature Reviews Endocrinology review reported that only 5% of neuroscience studies published in 2019 directly investigated the influence of sex. Those figures do not show that every older result is wrong; they show why many mechanisms and possible sex-specific effects remain under-tested.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. Why animal experiments provide stronger causal tests
Animal models allow researchers to manipulate variables that cannot ethically or practically be separated in people. Investigators can compare animals with different sex-chromosome complements while holding gonadal type constant, remove or replace gonads, administer hormones at defined developmental stages, or raise animals in controlled environments. They can then measure cellular, synaptic and behavioral outcomes.
Such designs can reveal whether a difference persists without gonadal hormones, appears only after a hormone exposure, or depends on a particular experience. They also show how timing works: the same steroid can organize a circuit early in life and merely activate it later.
Animal causality has limits. Species differ in brain anatomy, reproductive biology, social behavior and developmental schedules. A mechanism that is clear in a rodent or bird may not produce the same trait in humans. Animal findings are therefore strongest for identifying possible pathways, not for predicting a universal human “male brain” or “female brain.”

Are observed differences biological or just stereotypes?
They can be biological without being fixed, categorical or behaviorally decisive. A measured difference may reflect chromosome dosage, hormone exposure, experience or all three. Conversely, a stereotype is a social belief; it is not validated merely because a group average exists. The appropriate question is whether a specific trait shows a reliable, reproducible difference under defined conditions and what mechanisms explain it.
Most human traits are influenced by many genes and environments. Overlapping distributions mean that sex-linked averages have limited power to classify individuals. They also leave room for substantial change through learning, treatment, hormone changes, relationships and life events.
How to evaluate a claim about “sex differences in the brain”
- Specify the trait. Ask whether the claim concerns volume, connectivity, a hormone response, a cognitive task, a disorder risk or another measure.
- Check the population. Note species, age, developmental stage, sample size, geographic and social context, medication and endocrine state.
- Separate average from individual. Look for the amount of overlap, not only whether group means differ.
- Identify the design. A controlled animal manipulation can support causality; a cross-sectional human scan usually supports association.
- Ask which pathway was isolated. Did the study control chromosomes, gonads, hormones and experience separately, or could several have changed together?
- Look for replication and method sensitivity. Results that change with scanner, analysis, age or sample composition need cautious interpretation.
Why no single “brain sex” statistic is reliable
There is no universal percentage describing how different male and female nervous systems are. Effect size depends on the exact trait, sample, age, method and population. A single number would hide whether a result concerns a small overlapping anatomical measure, a hormone response in a narrow developmental window or a disorder risk shaped by many factors.
The most defensible conclusion is mechanistic: sex-chromosome genes, steroid hormones and experience all contribute, and their contributions are conditional. Human evidence supports average, probabilistic differences in some measures but does not justify two discrete brain categories.
