Some U.S. biomonitoring results show higher concentrations of selected chemicals in at least one lower-income subgroup—but they do not show that every low-income person is more exposed, or that the pattern still holds today. The clearest national analysis examined data collected from 2001 to 2008, and a chemical detected in blood or urine is not, by itself, evidence of harm.

What did the U.S. study find?

A peer-reviewed analysis of National Health and Nutrition Examination Survey (NHANES) data collected from 2001 through 2008 screened 228 chemicals. The authors could estimate at least one income-and-race subgroup comparison for 108 chemicals. Of those, 37 had at least one statistically different geometric mean ratio across the groups, and 12 had a ratio significantly higher than the study’s reference group in at least one subgroup. The study abstract and methods describe the analysis.

The reference group was non-Hispanic White participants with higher incomes. The researchers defined lower income as a poverty income ratio (PIR) below 2 and higher income as a PIR of 2 or more, and compared Mexican American, non-Hispanic Black, and non-Hispanic White participants across those income categories. The findings are subgroup comparisons—not a blanket result for all people with lower incomes.

The 12 chemicals with a higher ratio in at least one subgroup

  • Cotinine
  • Antimony
  • Lead
  • Thallium
  • 2,4-dichlorophenol
  • 2,5-dichlorophenol
  • p,p’-DDE
  • Methyl paraben
  • Propyl paraben
  • Mono-ethyl phthalate
  • Mono-isobutyl phthalate
  • Mono-n-butyl phthalate

The result does not mean every lower-income group had a higher level of each chemical. Nor does it identify why the subgroup differences appeared: the analysis does not isolate income as a cause or establish a single pathway behind the patterns.

Does this show that the pattern continues today?

No. The disparity analysis used NHANES measurements from 2001–2008, so its findings describe that study period. CDC’s National Report on Human Exposure to Environmental Chemicals covers nationally representative biomonitoring data from 1999–2000 through 2017–2018, but the report’s broader data span does not itself establish that the earlier income-stratified differences persisted. CDC explains the report’s scope and data periods; the available evidence does not settle whether newer research reproduces the income comparisons.

CDC says that typical chemical-measurement subsamples include about 2,500 participants per survey period, with exact counts varying by chemical and period. Those measurements are useful for characterizing selected exposures in the U.S. population, not for determining conditions in a particular neighborhood. CDC’s overview directs readers to tables for exact sample counts.

Does finding a chemical in blood or urine mean it is harmful?

No. Biomonitoring measures a chemical or its metabolites in blood, serum, or urine. Detection shows that exposure occurred; it does not, on its own, establish that the chemical caused disease, identify the source, determine a dose-response relationship, or predict an individual’s future health. CDC cautions: “Finding an environmental chemical in a person’s blood or urine does not mean that it causes health effects or disease.” For most chemicals, the effects of low-level exposure remain unknown. CDC explains what biomonitoring can and cannot show.

Can national data tell you what is happening in your community?

Not from NHANES national estimates alone. The survey is designed to characterize the U.S. population, not to estimate exposure for a state, city, county, or specific site. CDC’s national report also does not assign a detected chemical to a particular product or environmental source, make toxicity judgments, or provide regulatory recommendations. Community-specific measurements and studies are needed to answer local questions. CDC outlines these geographic and interpretive limits.

What does earlier environmental-equity evidence add?

EPA’s 1992 environmental equity report provides historical context, not a current nationwide estimate. It described higher-than-average exposure for racial minority and low-income populations in selected settings, including air pollutants, hazardous-waste facilities, contaminated fish, and agricultural pesticides in workplace settings. It also noted limited information on environmental health effects broken down by race and income. EPA’s report summary should be read as historical evidence rather than proof of present-day exposure levels.
