Antibiotics are the foundation of infective endocarditis treatment, but they are not always enough. Doctors add valve surgery when infection has damaged the heart enough to cause heart failure, cannot be controlled with antimicrobial therapy, has spread around the valve, or creates a substantial risk of septic emboli. In many serious cases, treatment includes both antibiotics and surgery. A specialist Heart Valve Team uses blood cultures, echocardiography, the infected valve, the organism, heart function and the patient’s stability to decide whether surgery is needed and how quickly it should occur.

Why antibiotics and surgery are not an either-or choice

Infective endocarditis is an infection of the heart’s inner lining, usually involving a valve. Bacteria or, less often, fungi can form infected growths called vegetations on valve tissue or implanted material. Those growths can destroy valve tissue, obstruct blood flow, break away and cause emboli, or extend into the tissue surrounding the valve.

Antibiotics attack the microorganism in the bloodstream and on the valve. Surgery repairs or replaces damaged valve tissue and removes infected or dead tissue that drugs may not be able to sterilize. Consequently, a patient may receive intravenous antibiotics before an operation, continue them during the perioperative period and complete a longer course afterward.

The 2023 European Society of Cardiology (ESC) guideline summarizes the principal surgical reasons as “HF, uncontrolled infection, and prevention of septic embolization (in particular, to the CNS).” HF means heart failure and CNS means the central nervous system. The American College of Cardiology/American Heart Association (ACC/AHA) guideline similarly lists heart failure, persistent infection, abscess, heart block, highly resistant organisms and recurrent emboli with persistent vegetations among reasons to consider early surgery.

How doctors establish the treatment plan

The decision is made by a multidisciplinary Heart Valve Team, usually involving cardiology, cardiac surgery, infectious-disease specialists, imaging experts and other clinicians needed for the patient’s complications. The team continually reassesses the plan as cultures, scans and the clinical course evolve.

1. Confirming the organism

Multiple blood cultures identify the microorganism whenever possible. The organism determines which antibiotic is active, whether resistance is a concern and how long treatment is likely to continue. A culture that stays positive despite appropriate therapy is a warning that the infection may not be controlled or that another source needs to be found.

2. Defining valve and surrounding-tissue damage

Echocardiography shows vegetation size and movement, valve leakage or obstruction and the effect on heart function. Transesophageal echocardiography is often needed for detailed views, especially with prosthetic valves or suspected complications. Additional imaging may be used when an abscess, false aneurysm, fistula or prosthetic-valve dehiscence is suspected.

3. Checking the patient’s stability

Symptoms and signs of pulmonary edema, low blood pressure, shock or worsening heart failure determine how quickly the team must act. Kidney, liver, neurological and other organ problems also affect the safety and timing of antibiotics and an operation.

What antibiotic treatment usually involves

Antibiotics are selected for the cultured organism and its susceptibility pattern, the infected valve and whether prosthetic material or a cardiac device is involved. Exact drugs, doses and durations are individual prescriptions; they cannot be chosen safely from a general article.

Initial hospital phase

The 2023 ESC guideline describes an initial two-week in-hospital parenteral phase for acute infective endocarditis management. “Parenteral” generally means medication delivered by intravenous or another non-oral route. During this period clinicians monitor cultures, temperature, heart function, kidney function, drug levels when relevant and evidence of complications.

Transition to outpatient or oral therapy

Some carefully selected patients can complete part of treatment with outpatient parenteral therapy or oral antibiotics after clinical assessment. The ESC key points specify that this step should begin only after imaging shows no local progression or complications and the patient’s condition is stable enough for the proposed setting. Outpatient treatment requires reliable access to medication, laboratory monitoring and rapid evaluation if fever or other symptoms return.

When antibiotics alone may be sufficient

Antibiotics alone may be reasonable when the infection responds, valve function remains acceptable, there is no abscess or other local extension, heart failure is absent and the embolic risk does not create a surgical indication. Close clinical and echocardiographic follow-up remains essential because deterioration can change the plan.

When surgery is added

The ESC groups indications into three broad categories. A patient can meet more than one category, and the urgency depends on the specific findings.

Heart failure or poorly tolerated valve damage

Severe acute valve regurgitation, obstruction or a fistula can overwhelm the heart. Emergency surgery is recommended in specified aortic- or mitral-valve cases when the lesion causes refractory pulmonary edema or cardiogenic shock. Urgent surgery is recommended when severe valve dysfunction causes heart-failure symptoms or other evidence that the circulation is not tolerating the lesion.

Doctors look for breathlessness at rest, rapidly increasing oxygen needs, fluid in the lungs, falling blood pressure, poor organ perfusion and imaging evidence of severe leakage or obstruction. Stabilizing medicines and ventilation may be used while the operating team prepares, but they do not remove the mechanical problem caused by a destroyed valve.

Uncontrolled infection or spread around the valve

Surgery may be needed when infection continues despite appropriate antibiotics or when it has invaded nearby tissue. Important examples include:

  • An abscess or pseudoaneurysm around the valve
  • A fistula between heart chambers or vessels
  • An enlarging vegetation
  • Prosthetic-valve dehiscence, meaning the prosthesis is coming loose
  • A new atrioventricular conduction block, which can signal an abscess near the electrical system
  • Persistent positive blood cultures or sepsis after other infection sources have been evaluated

Highly resistant or particularly aggressive organisms can also shift the balance toward early surgery. Removing infected material may be necessary even when the antibiotic chosen from susceptibility testing is appropriate.

Preventing septic emboli

Vegetations can break off and travel to the brain, spleen, kidneys, lungs or other organs. The ESC recommends urgent surgery in specified situations involving a persistent vegetation of at least 10 mm after an embolic event despite appropriate antibiotics, or a vegetation of at least 10 mm together with another surgical indication. Size is not a stand-alone rule for every patient: mobility, location, prior emboli, organism, valve damage and operative risk all matter.

How urgent is valve surgery?

“Emergency” and “urgent” describe different time frames. The ESC defines emergency surgery as occurring within 24 hours and urgent surgery as generally within three to five days for acute-phase indications. These are guideline categories, not a promise that every patient will be operated on at exactly those times.

Emergency operation

Refractory pulmonary edema or cardiogenic shock from acute valve failure can require an operation within 24 hours, sometimes while intensive-care treatment is under way. The immediate danger is hemodynamic collapse, so waiting to complete a planned antibiotic course is unsafe.

Urgent operation

When severe valve dysfunction, uncontrolled infection or embolic risk is present without shock, surgery is commonly arranged within the next several days. Intravenous antibiotics begin immediately and continue while the team completes imaging, evaluates other organs and prepares the patient.

Temporarily deferrable surgery or no surgery

In a stable patient without a surgical indication, antibiotics and strict clinical and echocardiographic observation may allow surgery to be delayed or avoided. New fever, positive cultures, worsening valve function, a new conduction abnormality, an embolic event or heart-failure symptoms can convert that plan into an urgent one.

Antibiotics versus surgery: what each treatment can and cannot do

Aspect Antibiotics Valve surgery
Primary purpose Eradicate the causative microorganism Remove infected tissue and repair or replace a damaged valve
Best evidence of benefit Falling fever, clearing blood cultures and clinical improvement Relief of dangerous valve dysfunction, removal of uncontrolled infection or reduction of embolic risk
Typical setting Initial inpatient intravenous treatment; selected patients later transition to monitored outpatient or oral therapy Operating room and postoperative hospital care, with antibiotics continued as directed
Main limitation Cannot reliably reverse severe mechanical valve destruction or drain an abscess Does not replace antimicrobial therapy and carries operative and recovery risks
How the choice is made Organism, susceptibility, valve context, organ function and response Heart failure, local extension, persistent infection, embolic risk, stability and operative context

What can change the recommendation

The infected valve and implanted material

Aortic and mitral valve infections can produce different hemodynamic problems. Prosthetic-valve endocarditis and infection involving a pacemaker or other cardiac device often require specialized imaging and may make source control more difficult. The presence of foreign material, prior operations and the feasibility of reconstruction all enter the surgical discussion.

The microorganism

Organisms differ in virulence, resistance and ability to adhere to prosthetic material. Resistant organisms can make medical sterilization less reliable. Culture results also determine whether an apparently persistent infection reflects inadequate drug coverage, an abscess, an infected device or a separate focus elsewhere.

Neurological or other embolic complications

Brain, spleen, kidney and lung events require coordination between specialists. The team balances the danger of another embolus against bleeding and neurological risks around an operation, using current imaging and the patient’s neurological status rather than a vegetation measurement alone.

Operative risk and recovery capacity

Age, previous cardiac surgery, kidney or liver disease, frailty, cancer, stroke and other conditions affect operative risk. They do not automatically rule out surgery; they help the team compare the risk of operating now with the risk of leaving the infection or valve damage untreated.

How common are shock and early surgery?

The 2023 ESC guideline estimates that approximately 5–10% of people with infective endocarditis develop septic shock. This is a population-level complication estimate, not an individual’s personal probability.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The 2020 ACC/AHA guideline says early surgical intervention is needed approximately 50% of the time in its discussion of infection and damage to valve leaflets and nearby tissue. That figure should not be interpreted as a prediction for a particular patient: the actual decision depends on cultures, imaging, complications, comorbidities and the Heart Valve Team’s assessment.

What patients and families should ask

Because treatment can change quickly, ask the treating team for a concrete explanation of the findings and the next decision point:

  • Which organism grew from the blood cultures, and how does it determine the antibiotic?
  • Are cultures now negative, and if not, what sources of persistent infection are being investigated?
  • What does the echocardiogram show about leakage, obstruction, vegetation size and valve strength?
  • Is there an abscess, fistula, prosthetic-valve loosening or a new conduction block?
  • Is heart failure or another sign of poor hemodynamic tolerance present?
  • Does the case meet an emergency, urgent or currently deferrable surgical category?
  • If surgery is recommended, what valve repair or replacement is possible, and what risks are specific to this operation?
  • What monitoring, antibiotics and recurrence-prevention plan will be needed after discharge?

Recovery and follow-up after treatment

Completion of antibiotics does not end follow-up. The ESC recommends patient-centered recovery planning, including information about prevention and recurrence risk. Follow-up commonly includes clinical review, repeat blood tests and echocardiography when indicated, medication monitoring and dental or other source-control advice tailored to the cause of infection.

Report recurrent fever, chills, increasing breathlessness, fainting, chest pain, new weakness, confusion or other sudden neurological symptoms promptly. A new symptom can represent recurrent infection, valve dysfunction, heart failure or an embolic complication and should not wait for a routine appointment.

Bottom line for the treatment decision

Doctors do not choose between “antibiotics” and “surgery” by applying a single cutoff. Antibiotics treat the infection; surgery is added when the valve or surrounding tissue is failing, infection is uncontrolled or embolic danger is high. Emergency surgery may be required within 24 hours for shock or refractory pulmonary edema, while urgent surgery generally occurs within three to five days for other acute indications. Stable patients without those findings may receive antibiotics with close observation. The Heart Valve Team’s interpretation of cultures, imaging, clinical response and individual risk determines the safest plan.
