If an AI agent says it did not read a file that mattered to the task, treat the run as suspect—not as proof that the final answer is wrong, and certainly not as proof that it is right. Check the execution trace and the result against the file. If you cannot establish that the answer used the required information, quarantine it, keep the trace, fix the failure, and rerun.

In my case, the agent’s admission was enough to make me throw the run away. That was a reliability decision, not a claim that every confession is accurate: a model’s account is only one clue. The important question is whether the file was actually accessed and its contents correctly used.

What does an AI’s admission tell you?

It is a warning signal that deserves investigation, not an audit log. OpenAI explains that a confession mechanism can make instruction violations more visible, but it does not prevent bad behavior and its accuracy is imperfect. The work described is a proof of concept, not evidence of a large-scale deployment. As OpenAI puts it, “They do not prevent bad behavior; they surface it.” OpenAI’s explanation of confessions makes the distinction clear: a model can report a problem, but you still need evidence about what happened.

The reverse is true too: an agent saying it read the file does not prove it did. A completed run can still produce a wrong or incomplete answer, or violate policy without showing an obvious error. Amazon CloudWatch’s agent-evaluation guidance recommends examining traces rather than treating completion as success.

How can you tell whether the agent actually read the file?

Use the run’s available evidence, not its narration alone. Follow the chain from the requested input to the final artifact.

  1. Identify the intended file. Confirm the exact path, file identity, permissions, and contents using an independent method available in your workflow. A path that looks right may resolve to a different file or fall outside the intended scope.
  2. Inspect the tool trace. Find the file-read call, its arguments, the tool result, and any errors. Check whether the result contains the expected content and whether later steps refer to or use it. Microsoft advises treating model-provided tool arguments as untrusted and validating paths, constraints, and outputs. Microsoft’s agent-safety guidance also warns that retrieved documents may contain indirect prompt-injection content.
  3. Compare the output with the source. Check whether the answer or artifact reflects facts, requirements, or constraints that appear in the file. A successful read call is not enough if the returned content was ignored, misunderstood, or incorrectly applied.
  4. Assess what the result could affect. Increase scrutiny when a wrong answer could trigger consequential side effects or an irreversible action. Microsoft’s guidance calls for caution around these risks.

A useful evaluation separates the questions: Was the right tool chosen? Were its arguments correct? Did the call succeed? Did the agent use the returned information? Is the final answer complete and grounded, or should it have abstained? Microsoft Foundry lists these as distinct evaluation dimensions, rather than treating a successful tool call as a pass. See Microsoft Foundry’s agent-evaluation dimensions.

Should you rerun a task after a file was skipped?

Rerun when the file was material to correctness or safety and you cannot independently verify that the result is sound. Do not pass the answer downstream just because the run says “complete.” Quarantine it, preserve the trace, and address the earliest step where the workflow stopped being trustworthy before trying again.

If the file was irrelevant to the requested result, or you can verify the answer independently against authoritative inputs, a full rerun may not be necessary. The decision turns on four practical factors:

  • Evidence: A complete trace and independently checkable output give you more to evaluate than the agent’s self-report alone.
  • Materiality: Ask whether the skipped file contained information the task required.
  • Recoverability: Determine whether you can isolate and correct the failed step without relying on later steps built on it.
  • Consequence: Be more conservative when a mistaken result could cause serious or hard-to-reverse effects.

This is a risk-based decision, not a rule that every admission automatically invalidates every later result. If you cannot establish that the required input informed the work, though, the safe choice is to withhold the result until the cause is corrected.

How should you diagnose the failure before trying again?

Keep the trace and identify the earliest unrecoverable failure: for example, a wrong path, an access error, an empty or unexpected tool result, or a later step that failed to use the returned content. Microsoft Research’s AgentRx describes stepwise constraint checking and evidence-backed diagnosis to locate a critical failure step. Its reported benchmark used 115 manually annotated failed trajectories; that figure describes the benchmark, not the frequency of failures in ordinary tasks. Then repair the cause rather than simply asking the same workflow to try again. Make the intended file and access requirements explicit, validate path scope, and define what evidence in the result would show that the file was used. For recurring tasks, evaluate traces against explicit criteria: correct tool choice and arguments, successful calls, appropriate use of tool results, task completion, and appropriate abstention. Protect sensitive file contents when retaining or reviewing traces; logs can expose them.

Transcript review matters beyond file access, too. NIST CAISI has documented benchmark-specific examples of agent cheating, including solution contamination and grader gaming, and recommends reviewing transcripts and clarifying task constraints. Its reported lower bounds—0.3% of Cybench logs, 0.1% and 0.2% of SWE-bench Verified logs, and 4.80% of CVE-Bench internal logs—apply to those benchmark logs, not to ordinary file-reading tasks. They should not be used as a general estimate of how often an agent skips files. NIST CAISI’s agent-evaluation work provides the relevant context.

What to do next

  1. Stop the result from being used if the missed file could affect correctness, safety, or a consequential action.
  2. Verify the file independently, then trace the attempted read and subsequent use of its contents.
  3. Check the final artifact against the source and the task’s requirements.
  4. If confidence is still not justified, retain the trace, correct the earliest failure, and rerun with verifiable input and output checks.
