Before exposing an AI system to users or production data, enable seven classes of controls: input and output moderation, authenticated identities with rate limits, constrained prompts and responses, prompt-injection defenses, least-privilege tools with approval gates, monitoring and adversarial evaluation, and deliberate privacy and retention settings. Add acceptable-use rules and an incident-reporting path so someone owns decisions when the model behaves unsafely.

The right profile depends on the workload. A chatbot mainly needs content filtering, abuse limits, safe fallbacks and escalation. An agent needs those controls plus isolation between untrusted text and privileged instructions, tightly allowlisted tools, sandboxing and confirmation for consequential actions. An API needs per-user safety identifiers, auditability and a provider-specific data-control choice before launch.

Choose controls by workload and blast radius

Risk increases when model output can change something outside the conversation. A text-only chatbot can usually be stopped by refusing or ending a session; an agent may read a private file, send a message or alter a production record. An API may serve many applications and users, making abuse attribution and retention decisions essential.

Workload Controls to enable before launch Primary failure to prevent
Chatbot Input/output moderation, authenticated or attributable users, rate and volume limits, bounded input and output sizes, safe fallback responses, and human escalation for high-stakes topics Harmful, abusive or overconfident responses at scale
Agent Everything in the chatbot profile, plus structured inter-step data, tool allowlists, approval gates, isolated filesystem and network access, sandboxing, and trace evaluation Untrusted content steering a tool into data exfiltration or an irreversible action
API Per-user safety identifiers, abuse monitoring, request and response limits, audit logs, retention selection, and review of every security-sensitive tool call Unattributed misuse, uncontrolled cost or privacy exposure across clients

There is no universal numerical reduction in incidents or single “safe mode.” The setting profile must reflect the provider, model, data sensitivity, user population, tool privileges and applicable law.

1. Moderate both inputs and outputs

Run a content-safety check on user input before it reaches the model and on the generated response before it is shown or acted on. OpenAI recommends its free Moderation API or an equivalent filtering system; Google recommends safety settings, safe fallback responses and narrower task designs. Treat a moderation result as a routing decision, not as proof that a response is correct.

Define the intervention for each category

  • Block: refuse requests that violate your policy or present clear abuse risk.
  • Transform: remove sensitive fields or rewrite a request into a permitted, bounded form.
  • Escalate: send high-stakes health, legal, financial, self-harm or security situations to a trained human process.
  • Allow with limits: answer benign requests while restricting length, tools or external actions.

Keep a safe fallback response for blocked or uncertain cases. It should explain the permitted next step without revealing hidden policy text or sensitive classifier details. For high-stakes uses, review the final output before a person relies on it; moderation does not replace factual or professional review.

2. Require identity, attribution and abuse limits

Make each request attributable to a user, account or service principal. OpenAI describes KYC-style controls and constrained input and output sizes as ways to reduce misuse; Google recommends unique user identifiers and query-volume limits. The exact identity requirement depends on the audience, but anonymous access should be an explicit risk decision rather than the default.

Set practical limits

  • Issue a unique safety identifier for each user or calling application and pass it consistently to the provider when supported.
  • Apply per-user, per-IP and per-tenant request limits, with separate caps for expensive models or tools.
  • Limit concurrent requests, daily volume and maximum input and output size.
  • Detect bursts, repeated policy violations and attempts to rotate accounts; route them to review instead of endlessly increasing limits.
  • Protect credentials with short-lived tokens, server-side storage and key rotation. Never put a provider secret in browser code or an untrusted client.

Log the identifier, policy decision, model or workflow version and timestamp needed for investigation. Minimize the prompt and response content in those logs when it is not required for the incident record.

3. Constrain prompts, outputs and state

Free-form generation creates more ways to produce an unsafe or unusable result. Set a maximum input length and output-token limit that fit the task, then make the interface narrower wherever possible.

Prefer machine-checkable contracts

  • Use enumerated choices, validated fields and fixed response schemas instead of asking for arbitrary prose when a finite set of answers is enough.
  • Validate type, range, length and allowed values after every model call. Reject or repair data that fails validation; do not pass it directly to a tool.
  • Retrieve facts only from approved material when the task permits it, and show the user when the answer is limited to that corpus.
  • Separate user-visible text from control data such as tool names, account IDs and authorization decisions.
  • Keep conversation memory scoped to the user and purpose. Expire or delete state that no longer serves the task.

For multi-step workflows, define an input and output schema for every step. A later step should receive only the fields it needs, not an entire transcript that may contain instructions hidden in quoted or retrieved text.

4. Defend against prompt injection

OpenAI defines prompt injection as an attack in which untrusted text or data enters an AI system and attempts to override the system’s instructions. Web pages, uploaded files, retrieval results, email, code comments and tool output are all untrusted by default—even when they came from a source your application normally trusts.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Keep trust boundaries explicit

  1. Place developer or policy instructions in a protected channel that untrusted content cannot rewrite.
  2. Label external text as data, isolate it from privileged instructions and pass only the fields required for the next step.
  3. Extract those fields with a schema, then validate length, type, destination and authorization before using them.
  4. Do not let a model-generated URL, command, recipient, file path or account identifier become executable merely because it is well formed.
  5. When a request is ambiguous or a validation check fails, stop and ask for review rather than guessing.

Instruction hierarchy and delimiters help the model distinguish data from instructions, but they are not a security boundary by themselves. The application must enforce authorization and validation outside the model.

5. Put tools behind approval and least privilege

Give an agent the smallest set of tools and permissions needed for its job. Separate read access from write access, restrict resources by tenant and environment, and use independent filesystem and network boundaries where possible.

Require confirmation for consequential actions

  • Ask for a human confirmation before changing accounts, files, money, production systems or external communications.
  • Show the exact proposed action, target, parameters and expected side effect in the approval screen.
  • Use separate approval for a sequence of actions when the target or risk changes; one approval should not become a blanket grant.
  • Set timeouts, quotas and an emergency revocation path for every tool.
  • Fail closed when review, authorization or a dependency is unavailable. Do not silently downgrade to unrestricted execution.

Cybersecurity-sensitive workflows should review every proposed tool call. Record the input that led to the call, the authorization decision, the result and any follow-up action so an investigator can reconstruct the chain.

6. Add guardrails, monitoring and evaluations

Guardrails should cover both content and behavior. Sanitize inputs for personal data and common jailbreak patterns, enforce output schemas, and inspect whether the agent selected an allowed tool for the stated task.

Test normal, adversarial and edge cases

  • Build a representative set of real tasks, including multilingual and accessibility-relevant inputs if your users need them.
  • Add adversarial cases for instruction override, data exfiltration, malicious files, tool-result poisoning and repeated abuse.
  • Run the set whenever you change the model, prompt, retrieval index, tool, policy or safety threshold.
  • Use trace graders or an equivalent evaluation to score tool calls, intermediate decisions, refusals and final answers—not only the final text.
  • Alert on spikes in blocks, retries, token use, tool failures, human overrides and policy violations.

Keep evaluation data separate from production secrets. Red-team findings should become regression tests with an owner and a remediation deadline.

7. Decide what the provider may retain

Read the provider’s current data-controls documentation before sending production prompts. OpenAI states that abuse-monitoring logs may include prompts, responses and classifier metadata and are retained for up to 30 days by default, subject to legal or safety exceptions. That is a default maximum for those logs, not a promise that every product, region or account has identical handling.

Match the setting to the data

  • Standard monitoring: use when the default retention and review process is acceptable for the workload.
  • Modified Abuse Monitoring: eligible customers can request this option when standard monitoring creates a compliance or confidentiality problem.
  • Zero Data Retention: eligible customers can request processing designed not to retain customer data, subject to the provider’s terms and feature limitations.
  • Private Safety Processing: OpenAI describes this as automated safety review designed to avoid retaining customer prompts or responses while still performing safety checks.

Confirm eligibility, supported endpoints, regional processing, legal exceptions and operational trade-offs directly with the provider. Independently minimize what you send: redact unnecessary personal data, avoid putting secrets in prompts, set your own log deletion schedule and restrict staff access to retained traces.

8. Publish policy and a reporting route

Write acceptable-use rules in language users can understand. State prohibited uses, limits of automated advice, what data is collected, how long your system keeps it and how a person can report harmful output or abuse.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Assign an owner for triage, security incidents, privacy requests and high-stakes escalations. Define severity levels, response times, evidence to preserve and who can disable a model, tool or tenant. OpenAI describes monitoring, privacy safeguards, review processes and reporting as parts of a broader safety ecosystem; your own policy should connect those functions to named operational roles.

Implementation recipes

Launching a chatbot

  1. Classify the conversations you will support and identify topics requiring human escalation.
  2. Configure input and output moderation with block, transform and escalate paths.
  3. Require a login or another attributable identifier when abuse, personalization or sensitive data makes anonymity unsafe.
  4. Set per-user volume limits, maximum message length and output-token caps.
  5. Use a safe fallback for blocked, uncertain or unavailable responses.
  6. Run representative and adversarial evaluations, then monitor blocks, escalation and repeat-abuse signals after launch.

Building an agent

  1. Draw a trust-boundary diagram showing user text, retrieved material, model instructions, memory and each tool.
  2. Convert every inter-step handoff to a narrow schema and validate it outside the model.
  3. Allowlist tools and parameters, separate read and write permissions, and isolate filesystem and network access.
  4. Insert confirmation before account, file, financial, production or external-communication changes.
  5. Set execution timeouts, quotas and revocation, with a fail-closed path when approval is unavailable.
  6. Evaluate traces for injection resistance, tool choice, authorization and side effects, not just answer quality.

Exposing an API

  1. Authenticate callers and assign a stable safety identifier per user or application.
  2. Enforce request, concurrency, token and tool-volume limits at the gateway.
  3. Apply moderation, schema validation and abuse detection before and after model execution.
  4. Select a retention and privacy configuration appropriate to the data, documenting eligibility and exceptions.
  5. Keep audit records for policy decisions, model and prompt versions, tool calls and human approvals.
  6. Review security-sensitive tool calls individually and fail closed on ambiguous authorization.

Compare safety designs with the same questions

When choosing an architecture or provider, score each option against the same dimensions instead of relying on a single safety label.

Dimension Questions to ask
Content-risk coverage Can you moderate both inputs and outputs, set category thresholds and provide a safe fallback?
Injection and exfiltration resistance Are untrusted sources isolated, fields validated and tool destinations independently authorized?
Tool permissions and blast radius Can you enforce least privilege, sandboxing, quotas, revocation and confirmation?
Human-review burden Which cases require approval, and can reviewers see the exact proposed action?
Privacy and retention What prompts, responses and classifier metadata are retained, for how long and under which exceptions?
Auditability and evaluation Can you retain useful traces, run regression suites and grade intermediate decisions?
Deployment complexity What identity, gateway, policy, logging and operational ownership must your team maintain?

NIST’s AI security overlay work is implementation-focused guidance for assistants and agents, not a completed certification standard. Use it as a control-based comparison aid, then verify each control in your own deployment.

Common failure modes and fixes

  • The model follows text in a retrieved page: treat retrieval as untrusted data, extract only required fields and validate them before any tool call.
  • A blocked request is answered after retries: count retries against the same user and preserve the original policy decision instead of allowing an alternate route to bypass it.
  • A tool executes with missing approval: make approval a server-side authorization check and fail closed when the reviewer or policy service is unavailable.
  • Logs contain secrets or unnecessary personal data: redact at ingestion, minimize trace fields and enforce deletion and access controls.
  • Safety degrades after a model update: rerun representative and adversarial evaluations whenever the model, prompt, retrieval data or tool changes.
  • Teams cannot explain an incident: record the model and workflow version, safety decisions, tool parameters, approvals and outcomes with a stable request identifier.
