AI drug-response models and cell-based screens answer different questions. A cell assay exposes a defined biological system to a compound and measures what happens under stated conditions: cell type, dose, exposure time, medium, plate, and readout. An AI model estimates a likely response from previously measured data and other features. Its output is a prediction, not a new measurement.
The most defensible workflow uses both. Models can rank compounds, identify promising cell contexts, and suggest follow-up experiments. Cell assays test whether those predictions hold in the biological system of interest. The experimental results then reveal where the model transfers, where assay context changes the answer, and what data should be added next.
What each approach actually answers
Traditional cell-based screening measures an observed response
In a cell-based screen, researchers select a biological system—such as a cancer cell line, primary cells, or an organoid—apply a compound at one or more concentrations, wait a specified period, and record a readout. Depending on the assay, that readout might represent viability, proliferation, apoptosis, reporter activity, morphology, or another endpoint.
The result is conditional. A statement such as “compound X reduced viability” is incomplete unless it specifies which cells, dose, exposure time, assay chemistry, controls, and laboratory procedures produced the observation. Changing any of those factors can change the measured value.

AI models predict a response from prior data
An AI drug-response model learns relationships between features and previously observed outcomes. Inputs can include molecular descriptors, gene-expression profiles, mutation status, drug structure, dose, cell-line identity, and assay metadata. The model then estimates an outcome for a drug, cell context, or condition that may not yet have been tested.
That estimate is conditional on the model’s training data and the setting in which it is used. A high predicted sensitivity does not prove that the compound will work in a new cell line, laboratory, assay format, animal model, or patient. The prediction should be treated as evidence for choosing the next experiment, not as a replacement for the experiment.
Why confusing prediction with measurement causes errors
A measured response contains experimental noise and biological variation, but it is an observation from the tested system. A prediction contains model uncertainty, possible bias in the training data, and uncertainty about whether the new setting resembles the data used to train the model. Reporting both as if they were equivalent can make a model look more certain than it is or make an assay result appear more general than its conditions justify.

| Question | Cell-based screening | AI drug-response model |
|---|---|---|
| Primary output | Measured response under a defined assay condition | Predicted response, ranking, or probability based on prior data |
| Conditions represented | The tested cell system, compound, dose, time, controls, and assay protocol | Only the biological and assay contexts represented adequately in training data and model inputs |
| Untested compounds or conditions | Requires a new experiment | Can prioritize untested combinations, subject to transferability and uncertainty |
| Throughput | Limited by plates, reagents, instruments, quality control, and laboratory capacity | Once trained, can score many candidates computationally; data preparation and validation remain limiting |
| Mechanistic value | Can reveal phenotype and, with suitable readouts, pathway or morphological effects | Can identify associations and influential features; mechanistic interpretation is not guaranteed |
| Transfer to a new drug, cell type, laboratory, or assay | Requires testing in that new setting | Must be demonstrated with independent or prospective validation; performance can drop after a context change |
| Uncertainty | Estimated from replicates, controls, calibration, and statistical analysis | Requires calibrated uncertainty, applicability-domain checks, and independent experimental tests |
| Complex tissue behavior | Simple cultures may omit immune, stromal, vascular, and three-dimensional interactions; advanced systems can represent more of them | Cannot infer behaviors absent from its data; tissue-level predictions need tissue-level evidence |
No universal benchmark establishes one approach as the winner across therapeutic areas, drug classes, assay formats, or clinical outcomes. The useful comparison is whether each method is suited to the decision being made.
What cross-study evidence shows about transfer
Within-dataset validation can be too optimistic
Xia and colleagues’ 2021 cross-study analysis examined drug-response data from NCI60, CTRP, GDSC, CCLE, and gCSI. They caution that cross-validation inside one biological dataset can overestimate how well a model will perform on independent data. A random split may place very similar drugs, cell lines, or laboratory measurements in both training and test sets, making the task easier than a genuinely new prediction.

For a realistic estimate, the split should match the intended use: hold out drugs when predicting new drugs, hold out cell lines when predicting new cell contexts, and hold out studies or laboratories when predicting transfer across facilities. If several kinds of novelty matter, they should be tested separately rather than hidden inside one average score.
Assay differences change apparent agreement
In Xia et al.’s mapped cross-study comparisons, CTRP-to-CCLE response measurements had an R² of 0.68, while CTRP-to-GDSC measurements had an R² of 0.41. These are agreements between experimental datasets after linear mapping, not AI model accuracy scores.
The study’s dataset table reports that CCLE and CTRP used CellTiter-Glo, whereas GDSC used Syto60. The lower CTRP-to-GDSC agreement is discussed in the context of such assay differences. A model trained on one readout should therefore not be assumed to transfer unchanged to another readout merely because both are described as viability assays.
Training-data composition affects generalizability
In simulations using CTRP data, Xia et al. found that increasing drug diversity contributed more to generalizability than increasing cell-line diversity in that study setting. This is a useful design signal for that dataset and task, not a universal law. A project focused on rare cell states, tissue types, or patient-derived material may need a different balance of diversity.
Where AI models add the most value
Prioritizing compounds before testing
A model can rank a large library for a particular cell context, estimate which compounds are most likely to be active, or flag combinations for follow-up. This can reduce the number of experiments needed to reach an initial shortlist. The ranking is useful only if the model’s training data cover relevant chemistry and biology and if the top predictions are tested prospectively.

Choosing informative experiments
Instead of testing compounds arbitrarily, researchers can select experiments that distinguish competing hypotheses, probe uncertain regions of the model, or add missing drug and cell diversity. An uncertainty-aware design may prioritize predictions with high expected value of information rather than simply selecting the highest predicted response.
Integrating heterogeneous measurements
Computational models can combine molecular structure, transcriptomic profiles, genomic alterations, dose-response curves, and metadata in one analysis. That integration can expose patterns that are difficult to inspect manually. It does not remove the need to check whether the measurements were generated with compatible protocols or whether one data source dominates the result.
Where cell-based screening remains indispensable
Confirming that a prediction survives contact with biology
A prospective assay tests the exact compound, cell system, concentration range, and endpoint that matter to the decision. It can reveal activity, inactivity, toxicity, edge effects, contamination, and protocol-specific behavior that a model cannot observe directly.
Studying morphology and emergent behavior
Phenotypic changes such as cell shape, organization, differentiation, or interactions among multiple cell types may not be captured by a single numerical viability label. The virtual-cell review in npj Digital Medicine (2026) identifies tissue morphology and complex multicellular behavior as areas where experimental approaches or tissue-level models remain needed.
Generating data outside the model’s applicability domain
If a compound has a novel scaffold, a cell type is poorly represented, or an assay protocol differs substantially from training data, a new experiment supplies information that cannot be recovered by changing the model’s algorithm alone.

A practical combined workflow
- Define the decision. State whether the goal is to rank compounds for one cell line, predict activity in an unseen cell type, select combinations, or understand a phenotype. The definition determines the validation split and the assay needed.
- Specify the biological and assay context. Record cell identity, passage or source information, medium, treatment time, dose range, endpoint, plate controls, instrument, and laboratory. Treat these as model features or eligibility criteria, not incidental notes.
- Audit and harmonize the data. Check units, concentration conventions, replicate handling, missing values, batch effects, response normalization, and assay chemistry. Do not merge nominally similar endpoints without testing their agreement.
- Design leakage-resistant validation. Use drug-, cell-line-, laboratory-, or study-level holdouts that represent the intended deployment. Keep a final independent set untouched until model selection is complete.
- Train with an explicit applicability domain. Track which compounds, cell states, and assay formats are close to the training distribution. Produce uncertainty or out-of-distribution flags alongside each prediction.
- Choose experiments from predictions and uncertainty. Test high-priority candidates, uncertain cases, and representative negatives. Include technical and biological replicates, positive and negative controls, and the same endpoint used in the training data when possible.
- Compare predictions with measurements. Report calibration, ranking performance, error by drug and cell context, and failures by assay or laboratory. A single aggregate metric can hide systematic failures.
- Feed results back carefully. Add validated measurements, document protocol changes, retrain only after checking for batch and leakage effects, and repeat independent validation. Do not treat every failed prediction as a reason to tune until it passes.
Transfer risks to check before deployment
New drugs
Novel chemistry can fall outside the structural and pharmacological patterns learned from existing compounds. Examine nearest-neighbor similarity, mechanism coverage, and uncertainty; then test a chemically diverse prospective set rather than only the most favorable predictions.
New cell types or disease states
A model trained on established cancer cell lines may not transfer to primary cells, resistant subclones, or patient-derived systems. Differences in lineage, genotype, growth rate, and baseline viability should be treated as potential distribution shifts.
New laboratories and instruments
Plate layout, incubation, liquid handling, imaging, and normalization can introduce site-specific effects. A site-transfer study with shared reference compounds can separate biological differences from measurement differences.
New assay formats
Switching from one viability chemistry to another, or from a bulk endpoint to imaging, can alter scale and biological meaning. The CTRP, CCLE, and GDSC comparisons show why assay labels alone are insufficient to establish interchangeability.

Interpretability and mechanism
Cell assays can provide mechanistic clues when paired with pathway reporters, genetics, time courses, microscopy, or perturbation experiments. They still measure phenotype rather than proving a causal mechanism.

AI models may highlight genes, molecular substructures, or interactions associated with response. Feature importance is not automatically a causal explanation: correlated variables, batch structure, and preprocessing choices can produce persuasive but non-mechanistic signals. A mechanistic claim requires an experiment designed to perturb the proposed driver and observe the predicted consequence.
What organoids change—and what they do not solve
Three-dimensional organoids and other human stem-cell-derived systems can represent architecture and cell-state interactions that simple monolayers miss. They may therefore be appropriate when the question concerns tissue organization, barrier function, differentiation, or multicellular response.
They are not automatically a settled replacement for conventional screening. A 2026 review, Human organoids: Fit for drug discovery?, describes reliability and scalability as open questions, including robustness, throughput, and protocol standardization. Another 2026 review on organoids in drug development reports limited training and validation datasets, reliance in some machine-learning studies on conventional two-dimensional or animal-derived data, and a lack of transparent benchmarks and standardized metrics. Those limitations make independent validation especially important when an AI model is applied to organoid data.

Common failure modes and fixes
| Failure mode | Why it fails | Better practice |
|---|---|---|
| Calling a prediction a measured response | It hides model and transfer uncertainty | Label predictions, confidence or uncertainty, and experimental results separately |
| Randomly splitting near-duplicate data | Related drugs, cell lines, or batches leak across train and test sets | Use group-based splits aligned with the deployment question |
| Pooling assays without checking comparability | Different chemistries and endpoints can measure different phenomena | Model assay identity, harmonize cautiously, and test cross-assay transfer |
| Optimizing only for the highest predicted responders | Selection can amplify model bias and miss informative failures | Include uncertainty, diversity, controls, and representative negatives |
| Assuming more complex models guarantee better biology | Architecture cannot recover information absent from training data | Match model scope to available measurements and validate at the relevant biological level |
| Using one metric as a universal score | Average performance can conceal failures in specific drugs, cell types, or sites | Report stratified errors, calibration, ranking, and independent prospective results |
How to choose the next step
- Use cell screening first when the compound or biological context is novel, the endpoint is poorly represented in existing data, or the decision requires direct phenotypic evidence.
- Use an AI model first when many candidate combinations must be prioritized and the training data closely match the intended drugs, cells, assay, and endpoint.
- Use both in sequence when computational ranking can reduce the search space but the result will inform a high-consequence experiment or a new biological setting.
- Escalate to tissue-level experiments when morphology, cell-cell interactions, or organ function is central to the question and cannot be represented by the current model or assay.
The 2026 review on virtual cells puts the principle plainly: “It is important to emphasize that the use of virtual cells should always be closely integrated with biological experiments.” That integration is not a ceremonial final check; it is how a prediction becomes evidence, exposes transfer failures, and improves the next model.
