The organization deploying an AI agent should assign a person accountability for its behavior and outcomes—and explicitly name who owns the documentation the agent relies on. The agent’s operator, knowledge-base owner, and platform provider may share operational work, but using a managed service does not automatically transfer responsibility for the organization’s data, use case, or approval rules.
Who is accountable when an AI agent uses outdated documentation?
A human in the deploying organization should be accountable for decisions made by the agent, including decisions made across multiple autonomous steps. The Australian Government’s agentic AI guidance says a human should be assigned that accountability. It applies to Australian Government agencies; other organizations can use it as a governance reference, not as universally binding law.
Accountability does not mean one person must perform every task. Assign responsibility for source content, agent configuration, and review of consequential actions. In a small team, one person may hold several responsibilities; the important thing is to make the ownership explicit.

Documentation or subject-matter owner
Decides which page is authoritative, who updates it when a product or policy changes, and when an older page is superseded. AWS recommends recording deployed-agent owners and keeping specifications and policy documentation current. NVIDIA’s documentation guidance describes connecting product changes to documentation updates.
#1 Best Overall
Agent or system owner
Maintains the agent’s specification, source connections, retrieval settings, and evaluation cases. This owner should be able to trace what the agent can access and how the system is expected to behave.
Operator or reviewer
Checks sources and citations when a decision is sensitive, ambiguous, or consequential, and can intervene before a high-impact action. Australian Government guidance calls for human oversight and intervention pathways; Microsoft recommends human-in-the-loop gates for high-impact or irreversible actions.

Platform provider
A provider may operate parts of a managed runtime or supply safety controls, but that does not by itself make the provider responsible for the customer’s source quality, data, use case, or approval policy. Microsoft’s shared-responsibility model is illustrative: the actual division depends on the service terms and configuration.

Check freshness and retrieval quality as separate things. A recently updated document can still be the wrong authority, and a retriever can return an obsolete copy even when a current page exists. NVIDIA recommends measuring freshness as the delay between a product change and its verified documentation update, alongside retrieval accuracy, citation coverage, variant accuracy, and task completion. Its guidance does not set a universal freshness target.

- Choose an authoritative source. For each important fact or task, identify a canonical page and link to it rather than maintaining competing copies.
- Connect changes to updates. When a product, policy, or procedure changes, assign the documentation update and verify it before treating the new information as available to the agent.
- Check what is indexed. Audit the knowledge store for obsolete, duplicate, overlapping, contradictory, incorrect, or diffuse material. Salesforce recommends this kind of content review and testing retrievers against test or ground-truth data.
- Test real questions. For common and high-risk tasks, verify that the expected canonical page is retrieved, the answer uses the right product or edition, claims cite the current source, and the documented steps reach their stated success check.
- Test stale-content detection. Include an intentionally superseded fixture in a test or release gate and confirm that the system catches it. NVIDIA recommends a parity gate designed to detect an intentionally stale fixture.
- Keep provenance. Where supported, carry a source version or last-modified value with retrieved content and compare it with the authoritative source. This is a practical implementation choice, not a universal metadata standard.
Retrieval strategies can also be compared against the same query set and ground-truth evidence. Salesforce discusses comparing vector and hybrid search in its product context; that guidance does not establish one approach as universally better.
What should happen before an agent acts on retrieved information?
Treat retrieved documents, tool outputs, and messages from other agents as untrusted input—not as authority to override the agent’s instructions or the organization’s controls. Microsoft’s agent security guidance recommends this approach. A document can inform a decision without authorizing an action.

- Require the appropriate authorization and human approval before actions that write data, change access, make payments, alter production systems, or communicate externally.
- Define an intervention path for sensitive, ambiguous, or irreversible work, rather than relying on a reviewer to notice problems after execution.
- Record the retrieved source and version when an answer informs a consequential decision. Traceable records help show what the agent relied on and what it did.
- Remove outdated or irrelevant information from agent memory, and review the memory and source-update process as part of ongoing oversight.
Does responsibility change with IaaS, PaaS, or SaaS?
The operational split can change, but the organization still needs to identify what it controls and who owns it. Microsoft describes different responsibilities across infrastructure, platform, and software services, while retaining customer responsibilities such as data, authorization, and oversight. Its model is illustrative, so confirm the specific service terms and configuration rather than assuming that a provider handles source validation or approval.
| Deployment model | What to establish | Responsibility that remains with the deploying organization |
|---|---|---|
| IaaS | Which party operates each layer, including the runtime and connected tools. | Ownership of the organization’s data, use case, access decisions, source content, and human oversight. |
| PaaS | Which agent and infrastructure components the platform manages, and which configurations the customer controls. | Ownership of the organization’s data, authorization, source quality, use case, and approval policy. |
| SaaS | What the service operates and what the customer can configure, inspect, or audit under the specific service terms. | Ownership of the organization’s data, authorization, use case, and decisions about when human review is required. |
This comparison is a responsibility-checking framework, not a legal allocation. The precise split varies by service and configuration.

What is a practical ownership checklist?
- Name an accountable human for agent decisions and outcomes.
- Identify a content owner for each authoritative source and define how product or policy changes trigger documentation updates.
- Assign an agent owner who maintains the specification, connections, retrieval configuration, and test cases.
- Test retrieval with representative questions and ground-truth sources; track freshness separately from retrieval and answer quality.
- Audit indexed content for obsolete or conflicting material, and test that stale copies are caught.
- Set authorization, human-review, intervention, and audit-record requirements for consequential actions.
- Confirm which tasks belong to the platform provider and which remain under the organization’s control for the configured service.
NVIDIA’s documentation guide says, “People remain responsible for decisions that create or change those rules.” That is the useful dividing line: automate retrieval and routine checks where appropriate, but keep ownership of authoritative content and consequential decisions with people.
