Forecasts identify where hunger is likely to deepen, when flooding could disrupt access, and what assistance is needed now. They do not show that famine has spread across East Africa, measure a single warning-to-action delay, or establish one reason for an under-response. The urgent question is how quickly funding, supplies and safe access can match the risks in each place.

What the latest forecasts say—and where

East Africa, the Horn of Africa and the World Health Organization’s “Greater Horn of Africa” are not interchangeable labels. The figures below come from different geographic groupings, dates and measures; they should not be added together or treated as one regional headcount.

WHO, August 2026–January 2027 strategic response plan Djibouti, Ethiopia, Kenya, Somalia, South Sudan and Sudan: more than 37.8 million people are experiencing or projected to experience acute food insecurity. More than 4.9 million children are projected to suffer acute malnutrition, including more than 1.5 million who require treatment for Severe Acute Malnutrition. These are the plan’s six-country Greater Horn figures for its response period, not a point-in-time count or a FEWS NET estimate.
Somalia joint release, 23 September 2026 IPC projects 5.4 million people in Somalia will face Crisis or worse hunger in October–December 2026, including 1.5 million at Emergency or worse. For July 2026–June 2027, 1.92 million children under five will require acute-malnutrition treatment, including 433,000 for severe acute malnutrition. These are Somalia-specific projections reported by the Federal Government of Somalia and the United Nations; the child-treatment figures cover a different period from the food-insecurity figures.
FAO–WFP Hunger Hotspots, released 17 June 2026 The global June–November outlook expected acute food insecurity to worsen in 13 countries. Sudan and South Sudan were among the most critical global hotspots by severity and magnitude, alongside Yemen and Palestine. This is global context, not a substitute for country-level forecasts in the region.

The IPC phases describe acute food-insecurity severity. A forecast of Crisis or worse is serious, but it does not mean every place in a country—or the whole region—is at the same phase.

Why the risk is severe, but not uniform

Conflict and economic pressure compound hunger

FEWS NET identifies conflict as the primary driver of acute food insecurity in its East Africa outlook. Displacement, weak macroeconomic conditions, high food and fuel prices, and weather shocks compound the impact. Those pressures differ by location, so the same regional label cannot tell a family’s risk or the response needed in a particular district.

Famine risk is conditional, not a regional declaration

FEWS NET describes credible but conditional famine risks in named areas of Sudan, South Sudan and Somalia. In Sudan and South Sudan, the risk scenario depends on conflict escalating or severe sieges returning and cutting people off from food and markets. In Somalia, it depends on extreme, widespread flooding damaging crops and isolating communities that are already vulnerable. These are scenarios about what could happen under specified conditions, not declarations that famine has occurred across East Africa.

Rain can bring relief and danger

In pastoral and agropastoral areas, FEWS NET anticipates above-average October–December rains that could replenish pasture and water. The same rainy season brings serious localized flood risks. In Somalia, the September joint release warns that the projected threat of El Niño-related flooding could further undermine fragile livelihoods, displace people and raise disease risk. A seasonal forecast therefore points to different outcomes depending on the location and severity of flooding.

What the Somalia update adds

The 23 September joint IPC release provides a more recent country-specific view than the broader outlooks. It reports that conditions in Burhakaba improved after Gu rains and a scaled-up response. At the same time, it identifies famine risk among displaced people in Baidoa based on April 2026 data; a follow-up assessment was planned for October. The dated basis matters: the Baidoa risk assessment should not be mistaken for a new October finding.

This contrast is a practical warning against treating forecasts as fixed outcomes. Rainfall and assistance can improve conditions in one area, while displacement, conflict or access constraints leave others exposed. The relevant question is not only how many people are at risk, but where, during which period, and whether assistance can reach them.

What acting early requires

Early warning does not itself deliver food, cash, treatment or safe passage. The official recommendations call for resources and operational preparation to be in place before conditions deteriorate or roads become impassable.

  • Keep food and cash assistance available. Maintain support for households facing acute food insecurity rather than waiting for conditions to worsen.
  • Fund nutrition treatment. Sustain treatment for severe and moderate acute malnutrition, including services for children who need urgent care.
  • Move supplies before access is cut. Pre-position food, emergency livelihood support and nutrition supplies ahead of flood-related road closures.
  • Make cash delivery ready. Strengthen cash-response readiness so support can be delivered when and where markets and access allow.
  • Reach communities through coordinated services. Expand food-security, nutrition, health, water and sanitation outreach in high-risk rural communities and displacement sites.
  • Maintain the systems behind response. WHO’s plan prioritizes surveillance and early warning, community protection, life-saving health and nutrition services, coordination and resource mobilization.

Preparedness is particularly time-sensitive where flooding could isolate people: supplies that exist elsewhere may not help communities cut off by damaged roads. The forecasts and recommendations establish the need to prepare, but the cited sources do not quantify a unified regional funding gap or say how much aid is currently reaching each area.

So why are we waiting?

The available evidence makes the question urgent, but it does not answer it with a single measured delay or a proven political explanation. The cited sources do not establish one regional decision maker or show how long it took for a warning to produce a response. They do show that agencies have specified likely periods, places and triggers, and are calling for timely financing, access and preparation.

That distinction matters for accountability. It is fair to ask whether assistance is funded and delivered in time, whether supplies are positioned before flooding, and whether people in high-risk areas can be reached. It is not supported by these sources to assign the lag to one actor or cause without separate evidence.

George Conway, Humanitarian Coordinator for Somalia, said: “The humanitarian crisis in Somalia remains dire and millions of people are at risk due to these high levels of hunger and malnutrition. The projected threat of El Niño-related flooding will further undermine already fragile livelihoods, displace vulnerable communities and increase the risk of disease. We call for urgent action and additional resources to protect lives, strengthen preparedness and prevent this crisis from deteriorating further.”

Etienne Peterschmitt, FAO Representative in Somalia, said: “Families already facing hunger cannot afford to lose their harvests or livestock to floods. Urgent funding is needed to turn early warnings into action, to save lives, protect livelihoods, and support early recovery for rural populations as this crisis deepens.”

The forecasts are not proof that every projected outcome will occur. They are specific warnings of preventable pathways: conflict and restricted access, worsening household food insecurity, malnutrition, and floods that can damage livelihoods or isolate communities. Whether those warnings become less severe depends on timely response in the places and periods identified—not on treating East Africa as a single uniform crisis.
