Boron is thought to be essential to the parasite-killing activity of acoziborole, a single-dose oral medicine being developed for gambiense human African trypanosomiasis (sleeping sickness). The European Medicines Agency (EMA) says the drug appears to interfere with a parasite protein involved in messenger RNA maturation, but the precise molecular interactions—including boron’s exact role—are not fully characterized.
What is the boron-containing drug?
Acoziborole is a benzoxaborole developed to treat gambiense human African trypanosomiasis (g-HAT), an infection caused by Trypanosoma brucei gambiense. Benzoxaboroles are a class of compounds that contain boron. The EMA’s 24 February 2026 opinion summary describes boron as thought to be essential to acoziborole’s trypanocidal, or parasite-killing, activity, while cautioning that the precise molecular interactions have not been fully characterized. How might boron help acoziborole work?
Available data suggest that acoziborole binds to cleavage and polyadenylation specificity factor 3 (CPSF3) in T. brucei. CPSF3 is involved in mRNA maturation—the processing needed for messenger RNA to function. Inhibiting this process can disrupt the parasite’s ability to operate. The EMA’s wording is deliberately qualified: this is the proposed mechanism, not a fully resolved account of every molecular interaction. In its 2026 opinion summary, the EMA reports treatment success 18 months after treatment for 159 of 167 people with second-stage disease (95.2%) and 41 of 41 people with first- or intermediate-stage disease (100%). The Phase II/III study took place in the Democratic Republic of Congo and Guinea; DNDi says its results were published in The Lancet Infectious Diseases in November 2022. DNDi summarizes success at 18 months across both stages as up to 96%. That summary and the EMA’s stage-specific figures use different presentations and should not be collapsed into a single statistic. DNDi: Acoziborole

Who is the medicine intended for, and how is it taken?
The EMA’s positive scientific opinion covers adults and adolescents aged 12 years and older who weigh at least 40 kg and have first- or second-stage T. b. gambiense infection, including severe second-stage disease. DNDi describes the regimen as three 320 mg tablets taken together as one oral dose. This indication does not mean the drug is appropriate for every type of sleeping sickness or every patient. What does the EMA opinion mean for access?
On 24 February 2026, the EMA’s Committee for Medicinal Products for Human Use adopted a positive scientific opinion for Acoziborole Winthrop through the EU-M4all process. The medicine is intended for use outside the European Union. The opinion can help streamline World Health Organization (WHO) prequalification and support national registration in endemic countries; it is not an EU-wide marketing authorization and does not mean the medicine is already available in every affected country. DNDi says Sanofi plans to donate the medicine to WHO for free distribution to patients. Actual access depends on the subsequent public-health and national registration steps in each country. Check the relevant national health authority or WHO for local availability. DNDi: Acoziborole
How does it fit with other sleeping sickness treatments?
Treatment choices depend on the parasite subspecies, disease stage and severity, and patient factors. The US Centers for Disease Control and Prevention (CDC) lists fexinidazole, nifurtimox-eflornithine combination therapy (NECT), pentamidine, suramin and melarsoprol among treatment options, with selection varying according to the infection and patient. CDC also advises monitoring for relapse for up to 24 months. Acoziborole’s single-dose regimen is a practical distinction for eligible patients with gambiense HAT, but it should not be used to infer which treatment is right for a particular person.
