The 1977 “Russian flu” was the return of human H1N1 after it had disappeared from circulation, and its virus closely resembled strains from decades earlier. That resemblance has led scientists to consider an accidental release, including a possible mishap involving live-virus vaccine work. But no reviewed evidence establishes exactly where the virus came from or how it reached people.
What was the 1977 “Russian flu”?
It was an outbreak of H1N1 influenza, a subtype that had stopped circulating in people after H2N2 replaced it during the 1957 pandemic. Edwin D. Kilbourne’s historical review says H1N1 first came to attention in the Soviet Union in November 1977, though reports later placed its first occurrence in northeastern China in May. “Russian flu” reflects the Soviet Union’s early report to the World Health Organization; it does not establish that the outbreak began there. The outbreak spread internationally and was unusually concentrated among younger people. A 2015 review describes cases primarily among people 26 and younger and characterizes the illness as unusually mild. One explanation for the age pattern is that people born after H1N1 disappeared had not previously encountered it. Zimmer et al., mBio, 2015
Why did the virus’s return raise questions?
The striking clue was its genetic resemblance to older H1N1 viruses. Zimmer and co-authors reported that the 1977 cluster’s hemagglutinin (HA) sequences were 98.4% identical to several strains isolated between 1948 and 1951. In their comparison, the HA protein differed at four of its 566 amino acids. Zimmer et al., mBio, 2015

That similarity points to a virus derived from an older lineage or stock, rather than ordinary, uninterrupted circulation through the years when H1N1 was no longer detected in humans. It does not reveal whether the virus was stored, used in an experiment, mishandled, or introduced by some other route. Genetic resemblance is evidence about the virus’s ancestry, not a record of its release.
Was the 1977 outbreak caused by a lab leak?
A laboratory accident is a plausible hypothesis, not a proven cause. The 2015 review notes that some isolates had temperature-sensitive traits discussed as possible signs of laboratory manipulation. At the same time, it reports that WHO investigators at the time reportedly ruled out a laboratory accident after discussions with Soviet and Chinese influenza laboratories. The review also notes that detections in three places in northeastern China do not fit neatly with a single laboratory accident. Neither observation settles the question. Zimmer et al., mBio, 2015

Could live-virus vaccine work have been involved?
Yes; it is another proposed route, not a documented explanation. Vaccine development and live attenuated influenza vaccine research were underway in the decades before the outbreak. Zimmer and co-authors report nearly 40,000 children across eight Soviet live-vaccine trials from 1962 to 1973, and also discuss trials in China. They raise the possibility that a vaccine or challenge virus was insufficiently attenuated or otherwise able to spread. The historical trial figures show that this kind of research took place; they do not demonstrate that a trial caused the 1977 outbreak. Zimmer et al., mBio, 2015

What can be concluded?
The strongest conclusion is that the 1977 virus was remarkably close to older H1N1 strains, while the outbreak’s origin and route remain uncertain. A laboratory accident and a mishap involving live-virus vaccine or challenge work are both hypotheses discussed in the literature; neither is proven. Kilbourne summed up the historical uncertainty in 2006: “Thus, the final answer to the 1977 epidemic is not yet known.” Kilbourne, Emerging Infectious Diseases, 2006
