If walking downhill brings on knee pain, shorten or pause the descents that aggravate it, keep activity within tolerable limits, and seek medical advice if the pain persists or comes with warning signs. Downhill pain is a trigger, not a diagnosis: several knee problems can cause it, and no single walking technique or aid is established here as a fix for everyone.

What to do when a descent hurts

  1. Reduce the provoking load. Shorten or pause descents that increase your pain. NHS self-care advice recommends putting as little weight as possible on a painful knee at first; return to downhill walking gradually as symptoms permit. This is a short-term adjustment, not a reason for prolonged inactivity.
  2. Keep movement appropriate to the cause. Treatment depends on what is causing the pain. If you have osteoarthritis, regular, consistent exercise is recommended even though it may initially cause some discomfort. NICE says this exercise will benefit the joints. That guidance is not a universal, unsupervised exercise prescription for every type of knee pain.
  3. Try simple comfort measures if appropriate. For knee pain, NHS guidance suggests applying a wrapped ice pack for up to 20 minutes every 2 to 3 hours. Rest, compression and elevation are also described by Mayo Clinic as self-care measures for an injured knee. Follow clinical advice if you have been given different instructions.

A clinician or physiotherapist can help tailor strengthening, flexibility, balance or movement-pattern work to your symptoms and diagnosis. There is not enough evidence in the cited guidance to recommend a particular stride, foot placement, cadence, backward-walking strategy or pole technique as a proven way to prevent downhill knee pain.

Why downhill walking may hurt

Knee pain can arise from injury, mechanical problems, arthritis or other conditions. Possible causes listed by Mayo Clinic include meniscal or ligament injuries, patellar tendon problems, patellofemoral pain, osteoarthritis and referred pain. The NHS knee pain guidance also lists sprains and strains, tendonitis, ligament or meniscus damage, osteoarthritis and bursitis. These possibilities cannot identify the cause from the downhill trigger alone.

When describing the problem to a clinician, note where the pain is, when it began, whether there was an injury, whether the knee swells, locks or feels unstable, and whether it also hurts on level ground or stairs. These details help put the downhill trigger in context.

Should you use a stick, poles, brace or insoles?

An aid may be worth discussing if pain or instability makes walking difficult, but the right option depends on the cause, terrain, duration and whether you can use it safely. NICE recommends considering walking aids such as sticks for people with osteoarthritis affecting a lower limb; that does not establish that trekking poles are better than a stick or specifically reduce downhill pain.

Brace or compression wrap Some conditions may call for a brace; compression may be part of self-care for an injured knee. Neither is a universal solution. Suitability depends on the condition.
Shoe inserts or arch supports May be considered in certain lower-limb osteoarthritis cases. Mayo Clinic describes arch supports as a possible way to shift pressure away from the more affected side of the knee in certain osteoarthritis cases. NHS guidance notes that footwear or insoles may be suggested for lower-limb osteoarthritis.
Cold pack For short-term comfort after a painful episode, if appropriate for you. NHS guidance suggests a wrapped ice pack for up to 20 minutes every 2 to 3 hours; it does not treat the underlying cause.

For osteoarthritis, NICE also recommends weight loss when excess weight is relevant: it can improve quality of life and physical function and reduce pain. This does not mean weight is a factor for every person with knee pain.

When to seek medical help

Get urgent advice

Seek urgent medical advice if the knee is very painful, you cannot move it or bear weight on it, it is badly swollen or changed shape, it locks or gives way, or you have a very high temperature with redness or heat around the knee. Mayo Clinic also flags inability to fully flex or extend the knee, deformity, and fever with redness, pain or swelling. Use the urgent-care route available where you live; NHS 111 and GP services apply in the UK.

Arrange a routine review

Arrange clinical review if the pain does not improve within a few weeks, affects sleep or daily tasks, or is limiting your usual activities. The NHS advises: “do not self-diagnose – see a GP if you’re worried.”
