Mandiant linked a 2022 espionage operation involving FortiGate firewalls, FortiManager, FortiAnalyzer and VMware systems to UNC3886, a group it suspects has a China nexus. The Fortinet flaw at the center of the report was CVE-2022-41328, a local FortiOS directory traversal vulnerability. Mandiant published its detailed account on March 15, 2023, so this is a historical incident—not a newly disclosed October 2026 campaign.
What was the Fortinet zero-day?
CVE-2022-41328 was a FortiOS directory traversal vulnerability. Mandiant reported that exploitation let attackers write files to FortiGate disks beyond the normal limits of shell access. Fortinet said it fixed the vulnerability in all supported FortiOS versions in its March 9, 2023 analysis of FG-IR-22-369.
The activity Mandiant investigated took place in mid-2022. The reports do not establish a campaign-wide victim count or quantify how prevalent exploitation was, so the incident should not be treated as a measure of risk across all Fortinet deployments.
Who did Mandiant link to the activity?
Mandiant attributed the activity to UNC3886, which it described as a group it suspects has a China nexus. That is an intelligence assessment, not a publicly established statement of nationality or government control. In its June 8, 2023 report, “Stealth Mode: Chinese Cyber Espionage Actors Continue to Evolve Tactics to Avoid Detection,” Mandiant also associated UNC3886 with a novel VMware ESXi hypervisor malware framework disclosed in September 2022.

How did the operation use Fortinet and VMware systems?
Mandiant’s account describes several linked stages across Fortinet products and VMware infrastructure, rather than a single firewall exploit. It reported persistence on FortiManager and FortiAnalyzer, with FortiManager scripts used to deploy backdoors to FortiGate devices. The compromised Fortinet systems were also involved in activity affecting VMware ESXi infrastructure.
Mandiant explicitly said it had no evidence, at the time of publication, that a zero-day was used to gain initial access or deploy the malicious VMware VIBs. The report therefore does not show that CVE-2022-41328 was the initial entry point in every affected environment.

Malware and persistence named in the reports
- Fortinet devices: Mandiant named THINCRUST, CASTLETAP, TABLEFLIP and REPTILE among malware families designed to interact with Fortinet systems. It described ICMP port knocking as a way to maintain persistent access on FortiGate, and custom API endpoints on FortiManager and FortiAnalyzer.
- VMware systems: Mandiant identified VIRTUALPITA and VIRTUALPIE as VMware backdoors.
- Firmware changes: Fortinet separately reported that modifications to device firmware could give attackers persistent access and control.
Why did some firewalls fail to start?
In this investigation, Mandiant reported that FortiGate devices with FIPS mode enabled failed a system checksum comparison after operating-system tampering and did not start. Fortinet’s analysis likewise said FIPS integrity checks detected changes and caused affected devices to halt or refuse to boot. The reported failures were a protective integrity response on affected devices—not evidence that all FortiGate appliances were compromised, or that FIPS mode prevents compromise.

For a historical vulnerability, the first step is to verify the FortiOS version running on each relevant device and follow Fortinet’s current guidance for that specific product and version. Fortinet said it fixed CVE-2022-41328 in all supported FortiOS versions; that statement does not determine whether a particular environment was compromised or whether every device is currently supported.

- Check product scope: Include FortiGate, FortiManager and FortiAnalyzer in an exposure review; Mandiant’s account involved all three product families.
- Review signs of unexpected activity: Investigate unexplained scripts or configuration and firmware changes, unusual persistence mechanisms, or activity linking Fortinet management systems to VMware infrastructure. The reports identify these areas of concern but do not provide a universal detection rule or prove that any one indicator confirms compromise.
- Escalate suspected compromise: Preserve relevant system and network evidence and involve qualified incident-response staff if findings suggest unauthorized access. Patching addresses the known vulnerability; it does not by itself establish that persistence or other malicious changes have been removed.
These steps are defensive guidance based on the products and techniques described in Mandiant’s March 15, 2023 report and Fortinet’s March 9, 2023 analysis; they are not a substitute for the latest vendor advisories or environment-specific investigation.
