Johns Hopkins researchers demonstrated a robot autonomously performing the clipping-and-cutting phase of gallbladder surgery on ex vivo pig tissue. It was not a complete gallbladder removal, was not performed on a person, and does not show that autonomous surgery is ready for patients.
Did an AI robot really perform gallbladder surgery on its own?
Yes, within a defined part of the procedure and a controlled experiment. The system, called SRT-H (Hierarchical Surgical Robot Transformer), autonomously carried out a sequence of tasks involving grasping tissue, applying clips to ducts and arteries, and cutting tissue with scissors. The Johns Hopkins-led team reported no human intervention during those autonomous test runs. The work was published in 2025 and described by Johns Hopkins as a realistic surgery demonstration (Johns Hopkins Malone Center, July 15, 2025).
“On its own” needs qualification: this refers to execution of that tested phase, not an entire operation with no human oversight. The project description says human voice feedback could steer or correct the system. The reported autonomous runs did not involve human intervention, but that does not mean there were no people involved in the broader research setting or that human supervision is unnecessary (SRT-H project page).
Was the procedure done on a human, and did the robot remove the whole gallbladder?
No. Tests used ex vivo pig gallbladders—tissue outside a living animal—not human patients. The demonstration focused on clipping and cutting. A complete cholecystectomy involves additional major steps that SRT-H did not perform.

- It did not dissect or skeletonize the hepatocystic triangle, the area where key ducts and blood vessels are identified.
- It did not dissect the gallbladder away from the cystic plate.
- It did not remove the gallbladder specimen.
Those distinctions are why “AI removed a person’s gallbladder” or “the robot completed a full operation” would be inaccurate. An independent 2026 clinical review details the omitted steps and the gap between the experiment and in vivo autonomous surgery (Limon et al., The American Surgeon, 2026).
How did SRT-H learn and control the robot?
SRT-H uses two linked policies. A high-level language policy plans the task and can issue corrective instructions when the system reaches a suboptimal state. A lower-level policy turns those instructions into robot trajectories. This separates deciding what should happen next from controlling the robot’s movements.

The team trained the system using demonstrations from surgeons operating on pig cadavers, with video and task captions. Training also included deliberately created recovery examples, such as missed grasps and misaligned grippers, so the system could learn how to respond when a maneuver did not go as intended. How often did it work?
The research team reported a 100% success rate across eight unseen ex vivo pig gallbladders. The tests varied conditions, including the robot’s starting position and tissue appearance altered with red dye; the team also showed corrective behavior after some misses (SRT-H project page). Eight successful specimens are evidence that the approach worked in those experimental conditions—not a clinical success rate, proof of safety, or evidence that it can handle the full range of human anatomy and surgical complications.

A separate number sometimes associated with this work concerns training data, not autonomous test outcomes: the 2026 ImitateCholec dataset paper describes more than 18,000 demonstrations from 34 ex vivo porcine cholecystectomies, approximately 20 hours of data, and 17 surgical tasks. Those figures describe a dataset supporting long-horizon imitation-learning research; they are not the number of autonomous SRT-H operations (Scientific Data, 2026).
What does the demonstration mean for surgical robots?
It is a meaningful step beyond a robot performing one isolated motion: SRT-H handled an extended, defined sequence of interdependent tasks. A 2021 review of surgical robotics distinguishes automation from autonomy and describes level 2 as a robot autonomously performing a surgical task; it also noted that most commercial platforms at that time had no decision autonomy (Attanasio et al., Annual Review of Control, Robotics, and Autonomous Systems, 2021). In that framework, the SRT-H result is best understood as task- or phase-level autonomy, not an autonomous operation.

The remaining clinical challenges are substantial. Bleeding, adhesions requiring adhesiolysis, obscured views, anatomical variation, and the unperformed stages of gallbladder removal all demand reliable recognition and response. Johns Hopkins identified testing other surgeries and expanding toward complete autonomous surgery as future goals; the 2026 clinical review emphasizes the gap between controlled ex vivo tasks and safe in vivo autonomy (Johns Hopkins Malone Center; Limon et al., 2026).
The sources describe a research proof of concept. They do not establish human-patient trial results, regulatory clearance for autonomous surgery, clinical outcomes, or patient availability.

How to compare claims about autonomous surgery
Not all “robot surgery” demonstrations show the same level of independence or capability. To judge a claim, check:
- Human role: Was a surgeon teleoperating, supervising, providing corrections, or absent during the tested execution?
- Task scope: Did the system perform one maneuver, a phase of a procedure, or every step of a complete operation?
- Test setting: Was the test in simulation, on ex vivo tissue, in a living animal, or with a human patient?
- Test breadth: How many cases were tested, and how much did the anatomy and conditions vary?
- Error handling: Could the robot recover from mistakes and cope with bleeding, poor visibility, adhesions, or anatomical variation?
These distinctions matter because success in a controlled animal-tissue experiment cannot, by itself, establish safe or effective autonomous surgery in people.
