Neanderthals were neither uniformly sick nor medically “modern” in any simple sense. Fossils show healed injuries, dental disease, joint degeneration and other physical effects of illness. Dental calculus preserves traces of food, smoke, plant compounds, microbial DNA and oral bacteria. Together, these clues support careful conclusions about diet, infection, survival and possible self-medication. They do not reveal pain, most soft-tissue disease, a complete lifetime diet, a doctor’s diagnosis or the exact treatment a person received.
The key distinction is between an observation and an inference: a remodeled fracture is directly visible; deliberate nursing, a splint or a medicinal preparation is a reasoned interpretation built around that observation.
What a Neanderthal fossil can actually record
Bone is living tissue. When it is injured or repeatedly stressed, it can remodel, leaving changes that may survive for tens of thousands of years. Teeth can preserve cavities, abscesses, wear and calculus (mineralized plaque). Calculus can trap microscopic particles and chemical residues, while ancient DNA from the same material can preserve fragments of oral microbes. These are different archives, each with its own strengths and blind spots.
A skeleton is also a filtered record. Many infections, fevers, nutritional deficiencies, cancers and neurological conditions leave no identifiable mark on bone. A rapidly fatal illness may kill before remodeling begins. Conversely, a visible lesion can have more than one cause. A researcher therefore has to state what is observed, how specific that observation is, how representative the sample may be and which alternative explanations remain plausible.

Bone evidence: injury, survival and long-term stress
Healed trauma
Healed fractures and remodeled lesions prove that an individual survived long enough for bone repair. They can also indicate the mechanical consequences of an injury: altered joint surfaces, asymmetry or degenerative change may show that a person lived with a lasting impairment.
Shanidar specimens in Iraq are central to this discussion. Several show antemortem trauma—damage that occurred while the individual was alive—and trauma-related degenerative joint disease. The remodeling demonstrates survival after serious injury. It does not, by itself, identify who helped, how often assistance was provided, whether a splint was used, or whether pain was treated with plants or other substances.
Dental disease
Teeth can preserve caries, abscesses, heavy wear and calculus. A dental abscess records infection in the tissues around a tooth, but not the sufferer’s subjective pain or the treatment attempted. Tooth loss and wear may affect chewing and therefore diet, yet they cannot be translated into a precise menu or a measured level of “health.”

Degenerative change and workload
Arthritic or trauma-related remodeling can indicate repeated loading or an old injury. It cannot distinguish a particular occupation, ritual activity or caregiving role without broader archaeological context. The same pattern may arise from several forms of stress, and severe disease can be absent from the skeleton if death occurred before bone reacted.

Did Neanderthals care for the sick?
Caregiving is inferred from the combination of injury severity, healing and the practical demands of survival. A person with major impairment who lived long enough for substantial healing may have needed food, protection or help moving through the landscape. Shanidar evidence is therefore compatible with assistance from other Neanderthals.
A 2018 review by Penny Spikins, Andy Needham, Lorna Tilley and Gail Hitchens examined survival with serious injury or impairment. The authors argue that Neanderthal medical treatment and healthcare was widespread and embedded in strong prosocial bonds. This is a behavioral inference, not a directly observed treatment protocol: no fossil records the identity of a caregiver, the schedule of care or the intention behind an action.
Different explanations can coexist. Practical cooperation might include carrying an injured person, sharing food, guarding a sleeping individual or helping with daily tasks. None requires a formal healer, and the evidence does not establish an institution resembling a clinic. It does indicate that at least some severely injured individuals remained alive long enough for social support to matter.

Dental calculus forms when plaque mineralizes on tooth surfaces. It can protect tiny particles and molecules from later contamination, making it unusually valuable in deep-time research. Karen Hardy and colleagues reported cooked plant foods, inhaled wood-fire smoke and bitumen or oil shale residues in Neanderthal calculus. Their 2012 conclusion was explicit:

Calculus can also retain ancient DNA and proteins. Laura Weyrich and colleagues sequenced calculus from five Neanderthals and found that regional dietary differences were associated with differences in oral microbiota. They described DNA preserved in calculus as a source of information about ancient hominin behavior and health and as a system for studying long-term microbial evolution.
These samples are not miniature diaries. Calculus usually represents a small, uneven portion of oral history. A particle may have been inhaled, eaten, accidentally incorporated or deposited from the surrounding environment. The material recovered has little relationship to everything an individual consumed. Cooked starches can be especially difficult to detect because salivary amylase breaks them down before preservation.
What Neanderthals ate—and why methods disagree
Dietary evidence varies by region and by method. A 2016 review documented 61 plant taxa from 26 families across 17 archaeological sites. That breadth argues against a single Neanderthal diet, but it does not mean every group ate every plant.

Nitrogen-isotope measurements place Neanderthals high in food webs, a pattern often associated with substantial animal-protein consumption. Tooth wear, starch granules and calculus residues reveal plant use that isotope averages may not show. These methods answer different questions: isotopes integrate aspects of longer-term trophic position, while calculus and wear provide more local or episodic traces. A high trophic position therefore does not rule out regular plant foods.
The El Sidrón case for possible self-medication
El Sidrón provides the strongest evidence compatible with Neanderthal self-medication. One individual had a dental abscess and a chronic gastrointestinal pathogen. Calculus evidence included poplar and antibiotic-producing Penicillium. The combination is consistent with ingesting plants or materials that could have had analgesic or antimicrobial effects.
The scientifically careful wording is “evidence compatible with self-medication.” The findings do not prove that the person recognized a pathogen, prepared a standardized dose, understood antibiotic mechanisms or followed a deliberate treatment plan. Poplar could have been encountered as food or fuel, and Penicillium may have been environmental contamination or an incidental food association. The abscess and pathogen make a therapeutic interpretation plausible, but they do not uniquely determine it.
This example illustrates why directness and specificity matter. A named microbial or plant signal is more specific than a general stress marker, yet the behavioral intent remains indirect. Multiple independent traces strengthen an interpretation without turning it into direct observation.

Ancient microbial DNA: a record of infection and preservation
Weyrich and colleagues reported a nearly complete draft genome of Methanobrevibacter oralis from Neanderthal calculus, dated to approximately 48,000 years ago, in a 2017 Nature study. This is the oldest reconstructed genome of that oral microbe described in the evidence summarized here.
The result demonstrates that microbial DNA can survive in protected dental deposits. It does not mean every Neanderthal fossil will yield a pathogen genome, nor does detection prove that a microbe caused illness. Many oral microbes are normal residents; disease depends on host condition, tissue location and interactions among organisms. A preserved sequence is evidence of past microbial presence, not automatically a diagnosis.
Four tests for judging a medical claim
When two interpretations compete, evaluate them on four axes rather than asking whether one sounds more vivid.

Regional ecology and preservation history are confounders. A cold steppe, a forested refuge and a cave with exceptional mineralization did not create the same archaeological record. Differences between sites may reflect behavior, environment, preservation or all three.
How healthy were Neanderthals overall?
The evidence cannot produce a reliable population-wide average for health, disease prevalence or life expectancy unless a study uses a representative sample and reports its uncertainty. Fossils are not a random census. Individuals with severe disease may be missing because they died before bone changes formed; individuals who survived long enough to heal are more likely to leave visible evidence. Archaeological recovery and preservation further skew what is available for study.
It is therefore reasonable to say that some Neanderthals survived serious trauma, experienced dental infection and degenerative change, consumed diverse foods and lived with complex microbial communities. It is not reasonable to convert those observations into a single health score or to claim that Neanderthals were generally healthier or less healthy than modern humans without a matched, representative comparison.
What fossils cannot tell us
- Pain or distress: Bone and calculus do not preserve a person’s subjective experience.
- Most soft-tissue disease: Many infections, cancers and organ conditions leave no skeletal trace.
- The complete diet: Calculus samples a tiny fraction of intake, and starch degradation removes part of the record.
- A clinician’s diagnosis: A lesion can have multiple causes and does not encode a diagnostic label.
- The exact treatment: Healing does not reveal whether someone used a splint, analgesic, plant preparation, surgery or only social assistance.
- Caregiver identity and motive: Survival may imply support, but fossils do not identify who provided it or why.
- Universal behavior: A signal from one cave or individual cannot stand for every Neanderthal population.
How to read a new claim about Neanderthal medicine
- Identify the material. Ask whether the claim rests on bone, tooth wear, calculus, isotope ratios, ancient DNA or a combination.
- Separate observation from interpretation. “Remodeled fracture” is an observation; “caregiver used a splint” is an interpretation.
- Check the sample. Note the number of individuals, sites, dates and preservation conditions.
- Look for alternatives. Food residue, smoke, environmental contamination and ordinary microbes can mimic therapeutic evidence.
- Match the strength of the language. Use “shows” for direct observations, “supports” for converging evidence and “is compatible with” when intent remains uncertain.
Bottom line
Neanderthal fossils show real disease and injury, but they show outcomes more often than causes or treatments. Healed trauma at Shanidar demonstrates survival; it supports, but does not prove, caregiving. Dental calculus reveals regional diets, plant residues, smoke exposure and ancient oral microbes. At El Sidrón, poplar, Penicillium, a dental abscess and a gastrointestinal pathogen make self-medication plausible without proving a formulated medicine. The most defensible picture is of adaptable people who sometimes survived serious impairment, used varied foods and lived within prosocial groups—not of a population whose medical system can be reconstructed in detail from fossils alone.
