Assess the specific AI system, task, data, people and workflow—not just the model’s reputation—before allowing workplace use. A defensible review assigns an accountable owner, maps intended and prohibited uses, tests performance and harms in representative conditions, adds controls for remaining risk, and defines when use must be paused or stopped. The National Institute of Standards and Technology (NIST) AI Risk Management Framework (AI RMF) organizes this work into four iterative functions: Govern, Map, Measure and Manage. NIST describes risk management as continuous throughout the AI system lifecycle; a vendor demonstration or general benchmark is not evidence that a tool is suitable for your particular workflow.

What a workplace AI risk assessment must establish

Before deployment, you should be able to answer five practical questions:

  • What exact task will the system perform, and what uses are prohibited?
  • Who operates it, who is affected by its outputs, and who can challenge or override them?
  • What evidence shows that it performs acceptably with your data, users and operating conditions?
  • What could go wrong—including privacy, security, discrimination, safety and business-continuity failures?
  • Who can pause the system, what event triggers that action, and how will you recover?

Risk belongs to the complete sociotechnical system: model, prompts, retrieval sources, data pipelines, interfaces, human reviewers, policies and surrounding decisions. The same model can present low risk when drafting an internal meeting summary and high risk when ranking candidates or recommending disciplinary action. Document the use case and consequences before discussing whether a particular model is “safe.”

Use NIST’s four-function lifecycle

NIST’s voluntary AI RMF is a practical organizing method, not a certification, legal safe harbor or guarantee of safety. Its functions are iterative: findings in testing can change the mapped use, which can require new governance and further measurement.

1. Govern the decision

Governance determines who is accountable before anyone turns the system on. Name a business owner who understands the intended process and an accountable decision-maker with authority to approve, restrict or reject deployment. Record the organization’s risk tolerance, approval thresholds, review cadence and escalation route.

Build a cross-functional review appropriate to the use. Procurement, information security, privacy, legal, HR or worker representatives, accessibility specialists and subject-matter experts may all be needed. Give each participant a defined responsibility rather than inviting general comments that no one owns.

  • Approval: identify who signs off on the mapped use, test evidence and residual risk.
  • Pause authority: specify who can suspend access immediately when a serious failure appears.
  • Training: train operators and reviewers on limitations, acceptable use, privacy handling and escalation.
  • Inventory: maintain a register of deployed and experimental AI systems, versions and owners.
  • Change control: require reassessment after model, data, vendor, workflow or legal changes.
  • Retirement: define how access, integrations, stored data and user instructions will be removed safely.

Governance should also prevent “shadow AI.” State which tools are approved, what information may be entered, and how employees can request a review for a new use. A policy that forbids all experimentation without providing a safe request path often drives unrecorded use instead.

2. Map the actual use

Write a plain-language use-case statement: “The system will [perform task] for [users] using [data] to produce [output], which will be used for [decision or action].” Add the limits: what the system must not do, which populations or situations are out of scope, and what a human must verify.

Your map should capture:

  • System, model and vendor names, versions and dependencies.
  • Purpose, expected benefit, operating environment and business process.
  • User groups, affected workers, customers, applicants or members of the public.
  • Input sources, data classifications, retention, transfers and access permissions.
  • Output types, downstream systems and whether outputs influence hiring, pay, scheduling, promotion, performance assessment or termination.
  • Human oversight: where review occurs, what the reviewer can change, and whether the reviewer has enough time, context and authority to reject the output.
  • Known limitations, exclusions, third-party services and failure dependencies.
  • Applicable jurisdictions, industry rules, collective-bargaining obligations and contractual constraints.

Distinguish assistance from automated or semi-automated decisions. A tool that drafts interview questions is materially different from one that scores applicants, even if both use the same underlying model. Describe the decision chain, not merely the interface: an apparently advisory score can become a de facto decision if managers routinely accept it without meaningful review.

3. Measure before launch

Test against acceptance criteria tied to the mapped task. Use representative data and realistic operating conditions, including difficult, incomplete, ambiguous and adversarial cases. Record the test design, sample limitations, results, uncertainty and who reviewed them. Quantitative, qualitative or mixed methods can all be appropriate; the method must fit the risk.

Measure more than average accuracy:

  • Validity and reliability: does the output address the task, and is it consistent across repeated runs and ordinary workflow variation?
  • Error patterns: what kinds of mistakes occur, how often, and how costly are they?
  • Robustness: what happens with missing fields, unusual language, changed formats, workload spikes or attempts to manipulate prompts?
  • Security and resilience: can unauthorized users extract information, alter instructions, poison retrieval sources or overwhelm the service?
  • Privacy: are sensitive inputs minimized, protected, retained only as needed and excluded from vendor training where required by contract or law?
  • Fairness: are error rates, access, recommendations or burdens different for groups affected by the decision?
  • Explainability and interpretability: can a reviewer understand the basis and limits of an output well enough to challenge it?
  • Accessibility and usability: can people with disabilities use the interface and receive an equivalent path to contest an outcome?
  • Human factors: do users recognize confident-sounding failures, or do speed and interface design encourage automation bias?

Use an independent reviewer when the impact, uncertainty or conflict of interest warrants it. A public benchmark may help compare models, but it cannot substitute for task-specific testing with your own workflow. Test before deployment and schedule regular testing in operation.

4. Manage residual risk

After measurement, decide whether remaining risk is acceptable for this use—not whether the model is universally safe. Tie each control to a documented failure or impact.

  • Restrict the use case, user group, geography, data fields or decision authority.
  • Require human review for specified outputs and prohibit automatic adverse action.
  • Add validation rules, source checks, confidence or uncertainty displays and structured output formats.
  • Mask or minimize personal and confidential information; limit retention and permissions.
  • Train users with realistic failure examples and require acknowledgement of operating limits.
  • Log prompts, outputs, reviewer actions, incidents and model versions where lawful and proportionate.
  • Monitor drift, complaints, disparate outcomes, security alerts and service failures.
  • Maintain an escalation, rollback and communications plan with named owners.

If residual risk exceeds the organization’s tolerance, narrow the task, add controls, choose another system or do not deploy. “Human in the loop” is not a sufficient control if the reviewer lacks authority, time, expertise or access to the evidence needed to disagree.

Build a complete risk inventory

Assess the system and workflow across the trustworthiness characteristics NIST identifies: validity and reliability; safety; security and resilience; accountability and transparency; explainability and interpretability; privacy enhancement; and fairness with harmful bias managed. No single test settles all of these.

Performance and operational failure

Define what a correct result means and what error is tolerable for the particular task. For a scheduling assistant, an error may create inconvenience; for a safety procedure or payroll calculation, the same type of error may cause material harm. Examine false positives and false negatives separately, because their costs are rarely equal. Establish a fallback process for unavailable, delayed or contradictory outputs.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Privacy and confidentiality

Classify every input and output. Check whether prompts, uploaded documents, logs or feedback are retained, used to improve a service, transferred across borders or accessible to subcontractors. Minimize data, enforce role-based access and provide a deletion or correction route where required. Do not place employee records, customer secrets or regulated data into an unapproved system merely because the interface appears private.

Security, resilience and abuse

Threat-model prompt injection, data poisoning, unauthorized retrieval, account compromise, model extraction, malicious outputs and denial of service. Test what happens when a document contains instructions intended to override the workflow, when a connected source is corrupted, or when the model is unavailable. Keep critical decisions operable through a documented non-AI fallback.

Fairness, accessibility and worker impact

Identify which groups may bear different error rates or burdens. Compare outcomes and error types across relevant groups using lawful, appropriate data and expert review; absence of demographic data does not prove absence of disparate impact. Assess language coverage, disability access, accommodation paths and the ability to challenge an output. Explain to affected people how the system is used and how they can request human review when applicable.

Transparency and accountability

Tell users when they are interacting with an AI system and what it can and cannot establish. Preserve enough documentation to reconstruct a material decision: input source, model version, output, human action and applicable rule. Allocate responsibility to people and teams; “the model did it” is not an accountability mechanism.

Additional checks for generative AI

NIST’s July 2024 Generative AI Profile is a cross-sector companion to AI RMF 1.0. It highlights governance, content provenance, pre-deployment testing and incident disclosure. Apply those concerns to generated text, images, code, summaries and recommendations.

  • Provenance: record which sources, retrieval indexes or uploaded files informed an output, and distinguish generated material from verified source content.
  • Fabrication: require task-specific verification for citations, numbers, identities, legal statements, code and other claims the model may invent.
  • Disclosure: decide when employees, customers or other affected people must be told that content or recommendations were generated or assisted by AI.
  • Prompt and output handling: prevent sensitive prompts from being reused in unintended contexts and scan outputs for confidential data, unsafe instructions and discriminatory language.
  • Incident learning: define what counts as a reportable hallucination, harmful image, data leak, security bypass or other event, and preserve evidence for investigation.

Generative fluency can increase over-reliance: a polished answer may receive less scrutiny than an obviously poor one. Include failure-recognition exercises in user training and make verification a required workflow step for high-consequence outputs.

Compare systems or workflows on equal terms

When several models or process designs could meet the same need, test them against the same task, data conditions, users and acceptance criteria. Record trade-offs rather than selecting the highest single score.

Comparison area Questions to answer
Task performance What are the error types, uncertainty, consistency and failure costs in the intended workflow?
Robustness and security How does each option behave with malformed inputs, adversarial prompts, outages and compromised sources?
Data governance Where do inputs come from, how are they retained, who can access them and can the vendor use them for other purposes?
Fairness and impact Are outcomes and error burdens acceptable across affected groups, languages and accessibility needs?
Human oversight Can reviewers understand, challenge and override outputs, and is review realistically resourced?
Transparency and auditability Are versions, logs, explanations, incidents and changes documented well enough for investigation?
Operations and recovery Can you monitor the system, receive incident notices, revert changes and operate without it?
Dependencies and cost What contracts, integrations, staffing, training, monitoring and exit work are required over the system’s life?

Make the trade-offs explicit in the approval record. A cheaper or more accurate system may still be unsuitable if it cannot support required oversight, retention controls or rollback.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Check legal and jurisdictional requirements

NIST AI RMF is voluntary guidance. It does not replace legal advice, sector regulation, employment obligations, collective agreements, privacy requirements or contract review. Have counsel assess the actual deployment, affected people and jurisdictions.

EU employment uses

The EU AI Act lists certain employment and worker-management systems as high-risk, including systems intended for recruitment or selection and certain systems affecting work-related decisions, task allocation, monitoring or evaluation. Classification depends on the precise intended purpose, the Act’s terms and exceptions, and whether profiling is involved. Do not classify every workplace AI application as high-risk without checking those details.

For a covered high-risk system used at the workplace, an employer deployer must inform workers’ representatives and affected workers before use. Deployers must monitor the system according to its instructions and keep automatically generated logs under their control for an appropriate period of at least six months, subject to applicable Union or national law.

The Regulation generally applies from 2 August 2026, while provisions have different dates and transitional rules. Check the current consolidated text, amendments, Commission guidance, national implementation, employment law and data-protection duties before reaching a compliance conclusion. A risk assessment is not a substitute for the required conformity, documentation or worker-information steps where those rules apply.

Create a deployment record before approval

Keep one controlled record for each system and material use. The following fields turn the assessment into an auditable decision rather than a collection of vendor slides:

  1. Identity: system, vendor, model and version; connected services; release and change dates.
  2. Purpose: intended task, expected benefit, prohibited uses and decision consequences.
  3. People: business owner, accountable approver, operators, reviewers and affected groups.
  4. Data: sources, classifications, permissions, retention, transfers and training use.
  5. Evidence: test data and methods, acceptance criteria, results, limitations and independent review.
  6. Controls: human-review points, access restrictions, validation, training, logging and incident routes.
  7. Legal context: jurisdictions, employment and privacy analysis, required notices and contractual terms.
  8. Monitoring: quality, security, privacy, fairness, complaints, drift and service-availability metrics.
  9. Decision: approved, approved with restrictions, deferred or rejected; record the rationale and residual-risk owner.
  10. Exit: pause triggers, rollback method, data disposition, communications and decommissioning criteria.

Mark which entries are measured evidence, which are contractual assurances and which remain assumptions. Assign a date and owner for every open issue; an unowned caveat is not a control.

Set go, restrict and stop thresholds

Approval should be conditional on observable evidence. Define thresholds before reviewing the final results so that enthusiasm for a promising demo does not silently change the standard. Examples include a maximum error rate for a task, zero tolerance for certain privacy or security events, a required completion rate for human review, or a complaint rate that triggers investigation.

Use three outcomes:

  • Go: acceptance criteria are met, controls are operating, owners are trained and residual risk is within tolerance.
  • Restricted go: use is limited by data, users, geography, decision authority or mandatory review while specified issues are addressed.
  • Stop or defer: evidence is insufficient, a critical control fails, legal status is unresolved or residual risk exceeds tolerance.

Write the pause trigger in operational terms: for example, a confirmed personal-data disclosure, unexplained disparity beyond the approved threshold, repeated unsafe recommendations, model-version change without retesting, or loss of the required human-review capacity. State who receives the alert and how users continue the work safely.

Monitor after deployment

Deployment is the beginning of operational evidence, not the end of assessment. Review performance and impacts on a cadence proportionate to the consequences and volatility of the system. Monitor model changes, data drift, workflow changes, user behavior, overrides, complaints, incidents, security events, accessibility issues and outcomes across affected groups.

Sample outputs for expert review, especially after updates or unusual events. Compare observed conditions with the assumptions in the original map. Reassess when the vendor changes the model, pricing or retention terms; when a new data source or integration is connected; when the task expands; or when laws and organizational policies change.

When an incident occurs, preserve relevant logs and inputs where lawful, stop harmful processing, notify the incident owner, investigate contributing workflow and human factors, and document corrective action. Resume only after the approval authority confirms that controls and evidence are adequate. If the system cannot be made acceptable, retire it and follow the documented data and access-disposal process.

A practical pre-deployment checklist

  • Is the exact task, benefit and prohibited use written down?
  • Are all affected people, decision points and jurisdictions identified?
  • Does an accountable owner have authority to approve and pause the system?
  • Are data sources, classifications, retention and vendor access understood?
  • Were representative, difficult and adversarial cases tested before launch?
  • Are error costs, uncertainty, robustness, security, privacy, fairness and accessibility documented?
  • Can reviewers meaningfully reject outputs, and are they trained and resourced?
  • Are generative outputs verified and provenance or disclosure requirements addressed?
  • Are legal, worker-information, contractual and sector obligations reviewed for each jurisdiction?
  • Are monitoring metrics, incident ownership, pause triggers, rollback and decommissioning documented?
  • Has residual risk been explicitly accepted, restricted or rejected by the authorized decision-maker?

If any answer is “not yet,” treat the deployment as incomplete rather than filling the gap with a vendor promise or a generic benchmark.

Frequently Asked Questions

Is the NIST AI Risk Management Framework mandatory?

No. NIST describes the AI RMF as voluntary, context-adaptable guidance. It can structure governance and evidence, but it does not replace legal, employment, privacy, security or sector-specific requirements.

Does human review make an AI use safe?

Not automatically. Review must be meaningful: the reviewer needs appropriate expertise, time, context, authority to override the output and a process for escalating failures. Otherwise, human review can become a rubber stamp.

When should an employer reassess an approved system?

Reassess after a model or vendor change, new data source or integration, expanded purpose, material workflow change, serious incident, emerging disparity, legal change or loss of required oversight capacity.
